Main duties of the job

As a Nurse in the frailty team, you will provide high-quality nursing care to frail and elderly patients, focusing on managing long-term conditions, wound care, and supporting proactive hospital discharge reviews. You will work within a multidisciplinary team, liaising with GPs, nurses, paramedics, pharmacy technicians, and voluntary services to ensure integrated care for complex patients.

About us

Townships 1 Primary Care Network is a forward-thinking PCN covering 5 GP practices that have worked hard to balance the benefits afforded by traditional ways of working in local practices with the innovation potential that working at a PCN scale brings. The PCN successfully applied to be a site for the NHSE PCN test site program, which started in October 2024.

Locally, there is a low turnover of staff in practices and the PCN. Townships 1 PCN has a wide variety of staff, including a well-established social prescribing team, a paramedic with an interest in advanced care planning, an OT who has led our proactive approach to frailty, pharmacy professionals, first-contact physio, health coaches, and effective care coordinator. There are strong relationships between the local practices and a substantial history of collaborative working.

The NHSE test site pilot will increase clinical staffing across the PCN by 10% and allow us to test new working methods. We are particularly interested in building on our approach to proactive care.
