Job summary

We have an exciting opportunity to join our in-house care navigation team here at West Leeds Primary Care Network to replace someone. This is a varied administrative role which is also patient facing via telephone. No two days are the same and if you like variety whilst making a difference in Primary Care this could be the role for you. This post would particularly suit someone who has worked within a GP or Primary Care network setting beforehand. Please see the uploaded job specification document for full details of the role

Main duties of the job

West Leeds PCN has taken over the responsibility of some of the administrative and care navigation elements previously undertaken by our member practices own staff in order to alleviate surgery pressure across our Network area (6 GP surgeries). With this in mind we created our own Booking Team who pick up multiple workflows previously completed by practice staff. We currently have two members of staff within the Booking Team and we are looking for a third to replace a staff member leaving.

About us

West Leeds Primary Care Network provides enhanced clinical services for six GP surgeries across the Pudsey and Bramley area, with a patient population of approximately 70k.

Contacting patients in a calm, empathic and professional way via various means such as telephone, letter, e mail or SMS etc to book them into clinics or to remind them about upcoming appointments.

Looking after a task box and making sure tasks are completed in a timely manner.

Answering the phone to patients to book them into the appropriate PCN clinic/s. This will require a good understanding of all of our services so that you can navigate to the right clinic.

Rearranging patient appointments promptly and sensitively in the case of unplanned clinical staff absences

Processing patient correspondence on SystmOne and entering the relevant clinical (SNOMED) codes

Data entry (for example covid vaccinations)

Assisting practices with their contractual obligations (ie QoF) in order to maximise practice income, for example running clinical reports, booking patients in for elements of healthcare and making sure they are coded thus afterwards.
