North East Derbyshire Primary Care Network Ltd is looking for a Community GP to join our dynamic and supportive Home Visiting Team. This is your opportunity to deliver high-quality, holistic care to housebound patients and those in residential or care settingswhile shaping the future of community healthcare.

  • 2 - 3 sessions per week, ideally Monday and Thursday

  • Salary: £10,920.00 per session per hour of 5 hours

Main duties of the job

What You'll Do

  • Provide clinical leadership and supervision to the Home Visiting Team
  • Conduct home visits for complex cases, including end-of-life care
  • Lead senior clinical triage and support the Local Navigation Hub
  • Work collaboratively across social care, ambulance, mental health, and community services
  • Deliver advanced care planning and reduce avoidable hospital admissions
  • Contribute to teaching, mentoring, and quality improvement

  • Demonstrable interest and experience in elderly care
  • Understanding of adult safeguarding and Deprivation of Liberty Safeguards (DoLS)
  • Experience in multidisciplinary team working and senior clinical triage

Desirable

  • Experience with Comprehensive Geriatric Assessment (CGA)
  • Experience in clinical supervision or mentoring
  • Familiarity with SystmOne clinical system

Qualifications

  • GMC registration with a licence to practise
  • Inclusion on the NHS England GP Performers List
