About Us

Mosaic Primary Care Network consists of 3 practices (Crown Heights Medical Centre, Odiham & Old Basing Health Centres and Bramblys Grange Medical Practice) located in or near Basingstoke. We have a combined patient population of circa 60,000 patients.

We are looking for an experienced paramedic to join our friendly team at Odiham and Old Basing Health Centres, ideally with a prescribing qualification.

The health centres are semi rural and are located in a small town and village, with staff working across both sites .

Main duties of the job

To provide a high-quality, cost-effective service to the patients of the PCN; diagnosing and, if qualified, prescribing treatments and medication while working within defined procedures and protocols as laid down by the GPs.

You will be working as part of a clinical team with GPs, Practice Nurses and supporting Healthcare Assistants.

Your primary task will be to work within the duty team, assessing and treating same day acute presentations. These will typically be patients presenting with a wide range of minor illness, urgent and same day requests for treatment. There is scope for chronic disease management in addition to same day care, depending on experience. You will be providing care for the presenting patient from initial history taking, clinical assessment, diagnosis, treatment and evaluation of care.

You will need to demonstrate safe, clinical decision-making and expert care, including assessment and diagnostic skills. You will need to prioritise and triage the needs of patients accordingly making any necessary referrals for investigations in the appropriate manner.

You may be asked to do home visits depending on need but the practice has a low home visit rate and is supported by a home visiting service, so the need will be minimal. You would not be asked to do palliative or end of life visits. Individual practices may wish to invest in training to enable the job holder to support the team in chronic disease management.

Job Responsibilities:

    • You will act as an autonomous clinician with the assistance of the clinical team
    • Work as part of an MDT within the PCN
    • Assess and triage patients, including same day triage and as appropriate provide definitive treatment (including prescribing medications following policy, PGD, NICE and local clinical guidelines and local care pathways, or make necessary referrals to other members of the primary care team.
    • Advise patients on general healthcare and promote self-management where appropriate, including signposting patients to the PCNs Social Prescribing service, and where appropriate, other community or voluntary services
    • Be able to perform and interpret ECGs, investigatory procedures as required and undertake collection of pathological specimens including blood sample, swabs and other samples within the scope of practice
    • Support the delivery of anticipatory care plans and lead certain community services
    • Provide an alternative model to urgent and same day GP home visit for the network and clinical audits
    • Communicate at all levels across organisations ensuring that an effective, person-centred service is delivered.
    • Communicate pro-actively and effectively with all colleagues across an MDT
    • Communicate effectively with patients, and where appropriate family members and their carers, where applicable, complex and sensitive information regarding their physical health needs, results, findings and treatment choices.
    • Ensure clinical practice is safe and effective, and remains within boundaries of competence
    • Clinically examine and assess patient needs from a physiological and psychological perspective, and plan clinical care accordingly
    • Take a comprehensive patient history, carry out physical examinations and use expert knowledge and clinical judgement to identify the potential diagnosis
    • Refer patients for investigations and/or the appropriate specialist where appropriate
    • Make a final diagnosis
    • Independently decide on and carry out treatment, including the prescribing of medicines, if qualified
    • Use your practice experience to plan and provide skilled and competent care to meet patients health and social care needs, involving other members of the wider health care team as appropriate
    • Ensure the provision of continuity of care including follow-up consultations / visits
    • Assess and evaluate, with patients, the effectiveness of the treatment and care provided and make changes as needed
    • Support patients to adopt health promotion strategies that promote healthy lifestyles, and apply principles of self-care

    • Participate in the administrative and professional responsibilities of the practices within the PCN
    • Ensure accurate and legible notes of all consultations and treatments are recorded in the patients computer record
    • Ensure accurate completion of all necessary documentation associated with patient care
    • Ensure collection and maintenance of statistical information required for regular and ad-hoc reports and audits
    • Attend and participate in clinical meetings
