Mosaic PCN has a growing team of pharmacists and pharmacy technicians who will benefit from a dynamic team leader who can provide support and leadership when required. There are opportunities to upskill and take on projects, as well as being part of a dedicated team . The role covers urban and rural patients so there is variation in the requirements of patients

About us

Mosaic Primary Care Network consists of 3 practices over 5 sites (Crown Heights Medical Centre, Odiham & Old Basing Health Centres and Bramblys Grange Medical Practice) located in or near Basingstoke. We have a combined patient population of circa 60,000 patients. We are looking for a lead pharmacist to join our expanding team.

The PCN is a registered learning environment, so all our ARRS staff are supported by an EELS lead to ensure we meet the aspirations of both the team member and the surgeries.

Job Overview

To improve health outcomes and the efficiency of the primary care network (PCN) pharmacy team by providing direct, accessible and timely medicines expertise as part of a multi-disciplinary team. To pro-actively complete workloads relating to medicines optimisation issues from other clinical staff to improve patient care, safety and outcomes.

To pro-actively complete workloads relating to medicines optimisation issues from other clinical staff to improve patient care, safety and outcomes.

The Lead Clinical Pharmacist must be a prescriber and registered with the General Pharmaceutical Council (GPhC). This role requires enrollment in (if not already qualified from) an approved 18-month training pathway or equivalent that equips the Clinical Pharmacist to be able to practice and prescribe safely and effectively in a primary care setting, for example, the CPPE Clinical Pharmacist training pathways.

The summary of the role includes:

a. work as part of a multi-disciplinary team in a patient facing role to clinically assess and treat patients using their expert knowledge of medicines for specific disease areas;

b. be a prescriber, or completing training to become a prescriber and work with and alongside the general practice teams;

c. be responsible for the care management of patients with chronic diseases and undertake clinical medication reviews to proactively manage people with complex polypharmacy, especially older people, people in care homes, those with multiple co-morbidities (in particular frailty, COPD and asthma) and people with learning disabilities or autism (through STOMP Stop Over Medication Programme)

d. Provide specialist expertise in the use of medicines whilst helping to address both the public health and social care needs of patients at the PCNs practices and to help in tackling inequalities;

e. provide leadership on person-centered medicines optimisation (including ensuring prescribers in the practices conserve antibiotics in line with local antimicrobial stewardship guidance) and quality improvement, whilst contributing to the quality and outcomes framework and enhanced services;

f. through structured medication reviews, support patients to take their medications to get the best from them, reduce waste and promote selfcare;

g. have a leadership role in supporting further integration of general practice with the wider healthcare teams (including community and hospital pharmacy) to help improve patient outcomes, ensure better access to healthcare and help manage general practice workload;

h. develop relationships and work closely with other pharmacy professionals across PCNs and the wider health and social care system;

i. take a central role in the clinical aspects of shared care protocols, clinical research with medicines, liaison with specialist pharmacists (including mental health and reduction of inappropriate antipsychotic use in people with learning disabilities), liaison with community pharmacists and anticoagulation; and

j. be part of a professional clinical network and have access to appropriate clinical supervision. Each clinical pharmacist must receive a minimum of one supervision session per month by a senior clinical / advanced practice pharmacist;

k. the senior clinical / advanced practice pharmacist must receive a minimum of one supervision session every three months by a GP clinical supervisor;

l. each clinical pharmacist/senior clinical pharmacist/advanced practice pharmacist will have access to an assigned GP clinical supervisor for support and development;

m. a ratio of one senior clinical / advanced practice pharmacist to no more than five junior clinical pharmacists, with appropriate peer support and supervision in place.

n.undertake clinical audits as agreed with the prescribing lead;

o.receive and resolve medicine queries from patients over the telephone and/or face to face,

p. lead and coordinate the PCN pharmacy team and be a point of access for any support they may require such as meetings, educational needs, professional discussions,

q. be responsible for workflow optimisation and delivery of daily services,

r. coordinate & deliver appraisals for the pharmacy team,

s. work alongside GP practice staff as an integrated part of the practice teams.
