Job summary

An exciting opportunity has arisen for a Clinical Pharmacist to join the dynamic and forward-thinking team at Healthier South Wirral PCN.

You will play a vital role in transforming patient care across our practicessupporting medication optimisation, leading Structured Medication Reviews (SMRs), and working closely with care home residents to improve health outcomes.

Main duties of the job

As a Clinical Pharmacist within Healthier South Wirral PCN, you will:

Conduct Structured Medication Reviews (SMRs) to improve outcomes and reduce medication-related harm.

Work closely with GPs, nurses, and other healthcare professionals to provide expert medication advice and support.

Support care home residents through regular visits and personalised medication reviews.

Lead on medicines optimisation, deprescribing, and repeat prescription management across the network.

Assist with long-term condition management (e.g., hypertension, diabetes, respiratory conditions) through evidence-based prescribing.

Contribute to clinical audits, QOF, IIF, and wider PCN service improvement initiatives.

Help to deliver patient education, support self-care, and promote shared decision-making.

This role is ideal for a pharmacist with strong clinical skills, excellent communication, and a passion for improving medicines optimisation in primary care.

About us

Healthier South Wirral Primary Care Network is made up of 5 Practices. Eastham Group Practice, Civic Medical Centre, Orchard Surgery, Spital Surgery and Sunlight Group Practice.

We currently have a list size of over 50,000 patients. The PCN also have a team of ARRS staff which includes Clinical Pharmacists, Mental Health Practitioners, Social Prescribers, Advanced Nurse Practitioners, Nurse Associates, Paramedics, a Trainee PA and Care Coordinators.

Job description

Job responsibilities

Conduct structured medication reviews -SMRs- for patients with long-term conditions, polypharmacy, or high-risk medications.

Identify and address medication-related issues, including adherence, side effects, interactions, and duplication.

Provide expert advice to GPs, nurses, and other healthcare professionals on medicines management and prescribing.

Work with a diverse range of patient groups, including individuals with long-term conditions, frailty, polypharmacy, and complex medication needs, to support safe, personalised care and improve understanding and adherence to prescribed treatments across the PCN.

Support safe prescribing practices in line with national guidance (e.g., NICE), local formulary, and evidence-based practice.

Assist with repeat prescription management, reducing inappropriate prescribing and waste.

Support the management of chronic diseases -e.g., hypertension, diabetes, COPD- through medication optimisation and monitoring.

The role also includes working closely with care home residents, conducting Structured Medication Reviews -SMRs- to ensure safe and effective use of medicines.

Work alongside clinicians to develop and implement care plans.

Participate in audits and quality improvement initiatives related to prescribing and medication safety.

Support practices in responding to MHRA alerts, drug recalls, and implementing safety protocols.

Maintain accurate and up-to-date clinical records.

Provide patient education on the appropriate use of medicines, including over-the-counter options and lifestyle advice.

Conduct clinics -e.g., anticoagulation, asthma, or hypertension- depending on training and competence.

Work across multiple GP practices within the PCN, contributing to integrated care delivery.

Collaborate with community pharmacies, hospitals, and other services to ensure continuity of care.

Participate in multidisciplinary team meetings and case discussions.

Supervise, mentor, or support the development of junior pharmacy staff or pharmacy technicians where applicable.

Participate in continuous professional development -CPD- and maintain professional registration with the General Pharmaceutical Council -GPhC-.

In addition to the primary responsibilities, the Clinical Pharmacist may be requested to:

Assist with the implementation and optimisation of clinical systems related to prescribing, medication alerts, and safety checks -e.g. EMIS, SystmOne-.

Use data and prescribing tools to identify patients for medication reviews, audits, and quality improvement initiatives.

Support QOF -Quality and Outcomes Framework-, IIF -Investment and Impact Fund-, and other performance indicators through pharmacy-led initiatives.
