Job summary

Wisbech Primary Care Network (PCN) is recruiting a Care Coordinator to support patient care in the development of the Primary Care Network.

The appointed Care Coordinator will primarily be based at Trinity Surgery. On occasions, there may be a requirement to work at other sites within the PCN, namely Clarkson Surgery, North Brink Practice and Parson Drove Surgery.

Main duties of the job

The Care Coordinator role is a critical and an evolving post with overall responsibility to help patients access the right services first time and will include:

Providing a point of contact for both staff and patients in coordinating care and will help to deal with incoming queries

Identifying patients from a task list to arrange follow-up appointments with the most appropriate clinician, liaising with our clinical teams or other outside agencies to implement treatment plans and ensure appropriate services to maximise the health and quality of life for the patients.

Providing administrative support to support the delivery of clinical work streams

Collating monthly patient utilisation data

Ensuring the directory of services remains up to date

About us

The PCN has a population of approximately 50,000 patients and comprises of 4 Practices North Brink Practice, Trinity Surgery, Clarkson Surgery and Parson Drove Surgery. The post holder will work alongside clinicians and other staff across the Wisbech Network as part of a multi-disciplinary team.

Job description

Job responsibilities

Run audits and searches where necessary to identify patients for review.

Serve as the point of contact for practice staff, providers, care teams, family/caregivers and community resources where indicated, responding with empathy and respect.

Help people manage their care needs by answering queries, making and managing appointments and ensuring that people have good quality written or verbal information relating to their care.

Arrange appointments or patients as directed by clinicians, following identification or urgent and non-urgent clinical needs.

Complete health questionnaires with patients to assess health status and risks of developing certain conditions, and refer where applicable to the relevant healthcare provider.

Refer patients to the appropriate team member and make referrals on behalf of the team.

Monitor referrals to ensure tasks are completed and care delivered by keeping in regular contact.

Signpost patients to appropriate community or local services.

Support the PCN to deliver quality metrics, such as QOF, KPIs and locally commissioned enhanced services.

Collating and reviewing the Impact and Investment Fund data

Where necessary, attend multi-disciplinary team meetings providing administration and documentation as required.

Document and monitor aspects of patient coordination and service delivery, supporting data collection and audit using the practices clinical system.

Work closely with and refer to PCN social prescribing link workers where a patient is identified as potentially benefitting from this service.

  • Ability to travel and work across multiple sites with use of own car (business use included on insurance policy)
  • Ability to work at other locations including home working if required
  • Willingness to work flexibly within the team to cover annual leave or sickness
  • Ability to travel within the requirements of the role
  • Meets DBS reference standards and has a clear criminal record, in line with the law on spent convictions
  • Access to own transport insured for business use and ability to travel across the locality on a regular basis, including to visit people in their own homes
  • Full driving licence with no restrictions and including business insurance cover
