This post is offered as a 12-month temporary contract; however, there is a strong likelihood that this role will become permanent as the team continues to expand and grow, reflecting the impact of the service and the ongoing support within our community.

Main duties of the job

This is an exciting opportunity to play a key role in improving health and wellbeing by working holistically with individuals to understand what matters to them. You will build trusted relationships, empower people to take greater control of their health, and connect them to community groups, voluntary organisations, and statutory services that provide emotional and practical support.

Working collaboratively across primary care and our Neighbourhood Health Partnership, you will help reduce health inequalities, support people to live independently, and improve overall health outcomes through personalised care and social prescribing.

Job responsibilities

Specific Responsibilities

Establish strong links and excellent working relationships with primary and secondary care colleagues, VCSE partners and other organisations.

Accept referrals and access how far a patients health and wellbeing needs can be met by services and other opportunities in the community.

Manage and prioritise your caseload reflecting your patients needs.

Together produce a simple personalised care and support plan to address their health and wellbeing needs by introducing or reconnecting people to community groups and statutory services, including weight management support, smoking cessation, taking up employment, training, welfare support (personal health budget) and sign posting where it matters to the person.

Sharing the knowledge and promoting the benefits with colleagues in the PCN on other services available in the local community and how this can be accessed

Support the VCSE organisation and community groups to become more sustainable, sharing intelligence regarding gaps or issues identified in local provision.

Gather data to build on evidence base about the benefits of social prescribing.

Ensure data is captured in the clinical systems.

Attend the peer support networks run by NHS England and NHS improvement at ICS level.

Be part of the Integrated Neighbourhood Teams to support patients and to link in with other organisations.

Have a proactive approach, the ability to work on your own initiative and within a team environment.

  • Enrolled, in progress or qualified from appropriate training as set out by the Personalised Care Institute.
