Main duties of the job

On a day-to-day basis, as a qualified or experienced paramedic, you will be autonomous and accountable, provide a holistic approach, undertake assessment, triage, management, treatment, and risk assessment underpinned by shared decision-making and the encouragement of self-management. In this role you would lead community-based health campaigns and anticipatory care approaches such delivering blood pressure monitoring clinics in sheltered housing. About us

Bridlington PCN is a forward-thinking network comprising of 2 practices over three sites within close proximity to each other with a total patient population of 42,000. Humber Primary Care and Drs Reddy and Nunn are both teaching practices linking into academic partners to provide ongoing support and development to the workforce.

Our aim is to improve care for patients by working collaboratively across primary care and our partners as part of the integrated care partnership.

Job Purpose

This role will provide a paramedic resource for health care professionals and patients by working collaboratively with other members of the multidisciplinary team, enhancing the capacity of our PCN.

Job Summary

To work within the locality, across the PCN, as an autonomous, accountable, paramedic providing a holistic approach for individuals including assessment, management, and treatment, to deliver high quality patient services. Key Areas of Responsibility

To work alongside the multi-disciplinary team across the PCN to assess and triage patients, including same day triage.

Provide definitive treatment such as the prescription of medications in accordance with guidelines (locally and nationally) and make referrals to other members of the primary care team.

Advise patients on general healthcare and self-management, including signposting patients to other relevant services across the PCN or wider, such as a Social Prescribing Link Worker.

Competently and with empathy, clearly communicate sensitive and/or complex information to patients and their families/carers too where relevant to ensure that they understand and can ask questions. This could be about their health needs, results, findings and treatment options.

Perform health checks and reviews that sit within your competence and guidelines, including ECGs (including the interpretation of these), investigatory procedures and collect pathological specimens such as intravenous blood samples and swabs.

Support the delivery of anticipatory care plans and take a lead in some community health promotion services such as blood pressure monitoring and diabetes risk assessments for people living in sheltered accommodation.

Provide an alternative model to urgent and same day GP home visits for the network.

Undertake clinical audits.

Collaboratively work with colleagues to maintain a person-centred service with a proactive approach to communication and a commitment to attend and contribute to discussions and meetings as required.

Ensure medical records are accurate and up to date and compliant with relevant legislation.

Embed evidence-based findings into developments and practice locally.

Ensure robust clinical governance is in place for your practice which includes regular audit and evaluation, supervision and CPD.

If working towards level 7 paramedic capability, work on a rotational model to ensure clinical supervision and support by a level 7 practitioner.

  • Educated to degree/diploma level in Paramedicine or equivalent experience
  • Registered with the Health and Care Professions Council (HCPC)
  • Has a detailed knowledge and understanding in practice of applying Caldicott Principles, Data Protection Act 2018 and appropriately protects an individuals personal data, disclosing where appropriate only
  • Critical understanding of theories and principles of enabling patients to improve their wellbeing
