Job summary

Whitby Coast and Moors Primary Care Network are looking to recruit an enthusiastic and compassionate Safeguarding Coordinator. Our PCN is setting up a new multidisciplinary holistic care team to provide dedicated support and care for patients with complex needs.

You will have the opportunity to share in the development of this new team which will focus on health, wellbeing and independence.

You will need to have clarity of professional identity and be comfortable working with a degree of uncertainty as new services are tested and developed.

Main duties of the job

The role of Safeguarding Coordinator is a new role within Whitby Coast and Moors PCN. It is envisaged the post holder will work closely with all PCN member practices to coordinate safeguarding work and create a safeguarding hub.

Whitby Coast and Moors PCN comprises 4 GP practices in Whitby and the surrounding area. Our member practices are Whitby Group Practice, Esk Valley Medical Practice, Staithes Surgery and Sleights and Sandsend Medical Practice. Our population faces particular challenges relating to higher than average chronic disease, coastal deprivation and rural isolation.

Job description

Job responsibilities

Responsibilities

Work with GPs to prepare safeguarding reports as requested from social workers ensuring the relevant consent is obtained

Prepare updates for child protection plan meetings, reply to invites and where appropriate attend these meeting (healthcare professionals only)

Work with GPs to prepare reports for MARACs and if appropriate attend MARAC meetings (healthcare professional only.)

Support and advise clinicians on safeguarding referrals

Occasionally discuss patients (where appropriate) with social services.

Develop a safeguarding hub for use by all clinicians across the PCN with resources to support referrals, contact information, training links

To work with the lead safeguarding GP for each practice to ensure that each practice's safeguarding policies are up to date and fit for purpose.

Ensure that practice staff are up to date with their safeguarding training.

Liaison with patients/families regarding any outstanding health issues raised through safeguarding conferences

o E.g. liaising with parents to book appointments for a childs health needs such as vaccinations or health reviews when a child is on a child protection plan

Maintain up to date registers/ coding of children on child protection plans and Looked After Children

Attend training courses organised by the Integrated Care Board for safeguarding coordinators.

Essential Skills

Excellent communication and interpersonal skills.

Excellent organisational skills

Good IT skills and ideally experience of using the clinical information systems EMIS and System One.

The coordinator will be required to have or obtain once in post level 3 safeguarding qualifications for healthcare professionals or level 2 safeguarding qualifications for non healthcare professionals.

Desirable Skills

Experience of working in safeguarding.
