We are looking for a proactive and digitally confident Digital Care Coordinator to join Jurassic Coast Primary Care Network, supporting three local practices: Ammonite Health Partnership, Barton House Medical Practice and Lyme Bay Medical Practice.
This is an exciting opportunity to play a key role in how healthcare is evolving locally. As digital tools, data and new technologies become central to modern healthcare, this role will help both patients and healthcare professionals make the most of these opportunities.
You will work closely with clinicians, administrators and PCN leadership to support the use of digital systems, population health data and innovative technologies such as healthcare apps, remote monitoring and shared care records. Your work will help identify patients who may benefit from proactive support, improve access to services and support the development of new ways of working across the network.
This role is ideal for someone who enjoys working with digital systems, data and people. You will help translate data and technology into practical action, supporting colleagues and patients to feel confident using digital healthcare tools.

Main duties of the job
The Digital Care Coordinator will support the development and delivery of digitally enabled care across Jurassic Coast Primary Care Network. The role focuses on helping both patients and staff make effective use of digital tools, data and systems to improve access to services and support better health outcomes.
The postholder will work closely with clinicians, administrators and PCN leadership to use population health data and clinical systems to identify patients who may benefit from proactive support or new services. They will also assist practices in embedding digital solutions into everyday workflows, ensuring systems are used efficiently and consistently across the network.
The role includes supporting colleagues with digital systems, helping maintain accurate datasets, assisting with service monitoring and providing practical support to PCN projects and service developments. The Digital Care Coordinator will also contribute to improving digital engagement by supporting communication through websites, social media and patient-facing digital resources.
This is a varied role that combines digital systems, data insight, administration and collaboration with a wide range of healthcare professionals to support the delivery of modern, patient-centred care across the PCN.
