Main duties of the job

The duties of the Advanced Clinical Practitioner (ACP) in the Ageing Well team involve providing high-quality, person-centred care using advanced clinical expertise. The ACP will conduct comprehensive assessments, undertake reactive visits, manage complex health needs, develop care plans, and prevent hospital admissions. The role includes leading on clinical governance, supporting frailty and complex care programs, and working collaboratively with multidisciplinary teams. Responsibilities also cover mentoring, service improvement, and promoting evidence-based practices to ensure the best outcomes for older adults in the community.

Jurassic Coast Primary Care Network (PCN) is a collaborative group of 3 GP surgeries located in rural West Dorset, including Ammonite Health Partnership, Barton House Medical Practice and Lyme Bay Medical Practice.

Serving a predominantly older population, the PCN focuses on delivering integrated, person-centred healthcare.

The PCN's vision emphasizes prevention, supported self-management, education, empowerment, and innovative care models. By working together, member practices aim to provide proactive, personalised, and coordinated health and social care to the local population.
