Main duties of the job
The duties of the Advanced Clinical Practitioner (ACP) in the Ageing Well team involve providing high-quality, person-centred care using advanced clinical expertise. The ACP will conduct comprehensive assessments, undertake reactive visits, manage complex health needs, develop care plans, and prevent hospital admissions.
