Job summary

Coventry North is looking to employ Advanced Nurse Practitioners to support the Primary Care Network team in the delivery of nursing services, working as part of the PCN multidisciplinary team, delivering care within their scope of practice to the entitled patient population. You should be an enthusiastic and highly motivated ANP with GP Practice experience. The successful candidate will be using advanced autonomous clinical skills and a broad and in-depth theoretical knowledge base in line with the latest RCN guidance on ANP competencies.

Main duties of the job

The Advanced Nurse Practitioner will work within their professional boundaries.

Providing care for the presenting patient from initial history taking, clinical assessment, diagnosis, treatment, and evaluation of their care.

To undertake surgeries working in an autonomous role including the management of minor ailments, chronic disease, family planning, home visits to include both acute visits and the management of chronic medical problems in the house-bound

To provide telephone-based triage, assessments, and consultations.

Demonstrate safe, clinical decision-making and expert care for patients within the general practice maintaining clinical excellence.

Work collaboratively with the multi-disciplinary general practice team to meet the needs of patients, supporting the delivery of policy and procedures. To work at this level, NMC requirements for advanced practice must be met.

Coventry North Primary Care Network are looking for a Paramedic Practitioner to join their team. Coventry North PCN serves a population of 50,000 patients.

We have an expanding range of clinical staff across the network and cover a wide geographical area. We have staff such as clinical pharmacists, Physician Associate, nurse associates and an extensive wellbeing team.

We offer supportive supervision for all new ARRS staff and hope to support them progressing forward in primary care.

Instigate necessary invasive and non-invasive diagnostic tests or investigation and interpret findings/reports

To prescribe safe, effective, and appropriate medication as defined by current legislative framework

Provide safe, evidence-based, cost effective, individualised patient care

Offer a holistic service to patients and their families, developing where appropriate an on-going plan of care/support, with an emphasis on prevention and self-care

Promote health and well-being using health promotion, health education, screening and therapeutic communication skills

Refer patients directly to other services/agencies as appropriate

Participate in developing and delivering specialist primary care services such as long-term disease management (diabetes, asthma, chronic obstructive pulmonary disease, coronary heart disease and hypertension).

Contribute to the practice achieving its quality targets to sustain the high standards of patient care and service delivery

Participate in identification of community health needs and develop patient/family-centred strategies to address them in liaison with appropriate community health professionals and agencies

Pro-actively promote the role of the Advanced Nurse Practitioner within the PCN Community ward and externally to key stakeholders and agencies

Identify appropriate opportunities to delegate both clinical and administrative tasks to more junior staff

Work closely with the doctors and administrative managers in the setting up and/or improving of practice systems for monitoring/measuring performance against Clinical Governance and Quality Indicator targets

Work collaboratively with colleagues within and external to the PCN

Communicate effectively with other team members, including the Community team, Social Services and Voluntary Organisations staffs as required.
