Job summary

The stop smoking practitioner is responsible for supporting and facilitating the delivery of smoking cessation programmes within the Healthy Communities integrated lifestyle service for people in Warwickshire

We have opportunities for new practitioners across Warwickshire, Clinics run from Monday Saturday 09.00 20:00 hours

Full and Part time roles available

A full driving license and business insurance is required due to the nature of the role

You will receive your own cohort of patients and Face to Face clinics including the delivery of group sessions to support and maintain and will help to improve the accessibility of Warwickshires Stop Smoking Service through the pro-active promotion of the smoking cessation programme and referral pathways working with key health care providers, allied professionals, community events, local agencies, and the general public.

This can be a challenging but extremely rewarding role for the right candidate. You will be part of both the smoking cessation team as well as the wider service team. You will also receive additional in-house training including data security, first aid and safeguarding.

The Ideal Candidatewould preferably have completed and be registered with the NCSCT as a Stop Smoking Practitioner already and have experience of working in this role.

Possess a minimum of 1 year experience working in a community-based smoking cessation service or Healthcare Setting.

Are experienced in using behaviour change models within a work context.

Can demonstrate a clear understanding of the range and appropriate use of different smoking cessation interventions.

Display strong knowledge of the harmful effects of smoking, smoking as an addiction behaviour, withdrawal, and relapse.

Are flexible with good time management and planning skills.

Are interested in motivating and supporting both individuals in virtual and community-based settings.

Can raise public awareness and provision of consistent healthy lifestyle messaging, on issues relevant to the health of the community and in a multi-cultural setting.

Have experience in building strong links and relationships with a diverse range of groups, Organisations, and agencies.

JOB DESCRIPTION

Stop Smoking practitioner

MAIN PURPOSE

Our Stop Smoking Practitioners deliver one to one and group stop smoking interventions within a wide range of settings. Across Warwickshire, our service will include work within one to one and group settings and also involve evening and weekend work.

The post holder will be required to work flexibly across the County as required ensuring their skills, expertise and knowledge are fully utilised.

This post will

Provide information to smokers to enable them to make an informed choice to make an attempt to stop smoking and then to support the smoker with a formal evidence based quit programme.

Develop and improve accessibility of the Warwickshires Health Communities Service and Stop Smoking Service through the pro-active promotion of the smoking cessation programmes and referral pathways amongst primary and secondary care services, other key health care providers, allied professionals, community development initiatives, local agencies and the general public.

Provide ongoing support to a caseload of referrals, supporting each quit attempt in line with national and company standards.

Contribute to the planning, delivery and development of the stop smoking service including specific project work.

In conjunction with the Delivery team lead plan and deliver an appropriate and accessible cessation service within the delivery team area.

Provide cessation support including one to one and group support, assessing clients suitability for Stop Smoking treatments.

Adhere to service protocols and procedures in relation to the provision of supportive pharmacotherapy medication.

Collate accurate clinical records in line with national PHE or NCSCT requirements and inputting data into the service web-based database in a timely and professional manner.

To support the service in delivering key performance indicators and activity as set out within the service specification.

Support the delivery of promotion activity across Warwickshire in line with the service marketing strategy.

Routine maintenance and the checking and calibration of carbon monoxide monitors in line with service schedules to ensure validity of quits in line with department of health guidelines.

Work flexibly to provide a coordinated service across the localities to meet the needs of the client group. This will include work in the evening and some work on weekends where required.

Maintain up to date clinical knowledge, undertake evidence-based practice and promote the use of new and innovative practice.

In conjunction with delivery team lead, conduct analysis and evaluation of designated programmes in order to improve the quality of service.

Be responsible for identifying issues or concerns to line manager, so that appropriate action can be taken

Contribute to ongoing needs assessment, audit and other governance programmes in relation to activities as part of the health improvement agenda.

Support additional service needs such as NHS Health Checks with Training if required.

In conjunction with the delivery team lead and Specialist advisor business & performance, marketing and remote access to support the promotion of the stop smoking service

Support level one advisors to actively promote stop smoking service within the delivery team area

Responsible for accurate uploading of own client data onto database whilst ensuring client confidentiality.

Ensure follow up of clients is done in line with service protocol

Keep accurate records in accordance with trust policy and data protection

To maintain an up to date electronic diary that is available to colleagues within the Stop Smoking team

To assist in producing reports and analysis of data that contribute towards improving service delivery and better client care

To utilise a range of complex motivational, influencing and negotiating skills to increase clients motivation and adherence to stop smoking.

Responsible for personally identifiable client data as part of ongoing programme attendance etc., working within the data protection policies.
