Job summary

Bexhill PCN is looking for a Community Paramedic to join our team.

Bexhill PCN works alongside the GP surgeries in Bexhill - Little Common and Old Town Surgery, Dr Warden and Partners, incorporating, Collington, Pebsham and Ninfield Surgery and Sidley Medical Practice, supporting our patients, reducing health inequalities and providing the best care to our patients.

The suitable candidiate would have patient care in their forefront, be proactive and a team player.

Main duties of the job

To Support our Enhanced Health in Care home work. In this exciting role, you will work alongside our PCN Advanced Clinical Practitioners, Care Co-Ordinators, as well as supporting the Bexhill GP Practices with general triage and clinical support.

No day will be the same. You could be at the PCN hub working with the team, out visiting patients in Care homes, or at their home, or supporting our local GP Surgeries.

Job responsibilities

Key duties and responsibilities

Thie successful applicant has to be a qualified Paramedic

- Provide clinical support to our Enhanced Health in Care home work. Work alongside the ACP's, Care Co-Ordinator's, Practice staff, Secondary Care and community staff to carry out Multi Disciplinary team meetings and support patients living in care and residential homes.

Advise patients on general healthcare and promote self-management where appropriate, including signposting patients to other community, voluntary service or ARRS Teams.

Be able to:

Perform investigatory procedures as required

Perform investigatory procedures needed by patients and those requested by GPs and the wider PCN team.

Support the delivery of anticipatory care plans and lead certain community services (e.g. Care home Patients and the Housebound )

Communicate at all levels across PCNs and other organizations, ensuring effective, patient-centered service

Communicate proactively and effectively with all colleagues across the multi-disciplinary team, attending and contributing to meetings as required and Lead of the PCN Multi-disciplinary meetings where required.

Collaborate with other members of the PCN including doctors, nurses and other AHPs, accepting referrals and referring to them for specialist care

Maintain accurate and contemporaneous health records appropriate to the consultation, ensuring accurate completion of all necessary documentation associated with patient health care and registration with the practice

Assess, diagnose, plan, implement and evaluate treatment/interventions and care for patients presenting with an undifferentiated diagnosis, and patients with complex needs

Clinically examine and assess patient needs from a physiological and psychological perspective, and plan clinical care accordingly

Prioritise health problems and intervene appropriately to assist the patient in complex, urgent or emergency situations, including initiation of effective emergency care

Diagnose and manage both acute and chronic conditions, integrating both drug- and non-drug-based treatment methods into a management plan

Support patients to adopt health promotion strategies that promote healthy lifestyles, and apply principles of self-car.

Administrative requirements

Produce accurate, contemporaneous and complete records of patient consultation, consistent with legislation, policies and procedures

Be aware of data protection (GDPR) and confidentiality issues particularly within a PCN

Use technology and appropriate software as an aid to management in planning, implementation and monitoring of care, presenting and communicating information

Review and process data using accurate read codes in order to ensure easy and accurate information retrieval for monitoring and audit processes.

- A driving licence is required.

The role is part time 25 -30 hours per week. Monday to Friday 8am - 6.00pm.No weekends, unless you wish to support our Extended Access Clinics.
