As a CYP Wellbeing Practitioner, you will play a crucial role in providing accessible and early intervention wellbeing support to children and young people (CYP), and their families through effective, evidence-based interventions.
In this dynamic role, you'll manage a diverse caseload across our three member practices within the Redhill Phoenix PCN.
You'll be expected to provide a dedicated weekly day at each site, where you can connect with patients either in person or remotely.

Main duties of the job
Working across
member practice sites to provide early intervention wellbeing support to deal
with mild-moderate emotional wellbeing needs of our CYP population via 1:1 or
group sessions
Identifying and responding to risk, following local child
safeguarding & escalation procedures as required and working with partner
organisations and services such as schools, CAMHS & VCFSE partners
Recording clinical
outcomes and report on KPIs to demonstrate service performance and support Quality
Improvement & service development

Job Summary
To provide accessible, early intervention wellbeing support for children and young people (CYP) and
their families/carers through brief, evidence based interventions delivered on site or
remotely.
As part of this role, you will be expected to collect, analyse and
report outcomes data, ensuring high data quality, robust governance, and
meaningful feedback that demonstrates value and drives continuous service
improvement and meets requirements set out by PCN member practices.
You will work across the 3 member practice sites with a weekly dedicated
day to each site.
Key Responsibilities
Key responsibilities will include but are
not limited to:
- Complete
initial assessments to identify, triage and respond to risk, following local
safeguarding children & escalation procedures as required
- Provide
early intervention wellbeing support where appropriate to support mild-moderate
emotional wellbeing needs via 1:1 or group sessions
- Involve
parents/guardians where appropriate and provide brief parental support aligned
to the CYPs goals
- Coordinate
with GPs, CAMHS, schools, and VCFSE partners; make timely referrals and
contribute to multiagency working where appropriate
- Record
clinical outcomes and report on KPIs to demonstrate service performance
- Uphold
data governance (IG/GDPR), collect consent, and maintain confidentiality
- Demonstrate
motivation, reliability, flexibility, and commitment to the role
- Establish
and maintain good working relationships with the GP practice teams
Meetings
- It may be necessary to attend and
contribute to various Practice, PCN and/or DHC meetings and forums
- To attend regular meetings with your
line manager, feeding back updates, issues or challenges
