Job summary
We are looking for an enthusiastic and caring candidate for the
post of Well-being Coordinator for Redhill Phoenix. The role requires
great communication skills, ability to use ones own judgment, and initiative.
You will be required to work 10-12 hours per week, some of
which can be remote but some will be in community settings.
Main duties of the job
The candidate will be required to work with the Redhill Phoenix
PCN practices. They will support and
establish community-led groups to improve their health and well-being, where a health need has been
identified.
The candidate will work in collaboration with the GrowingHealth Together
team and will spend time on outreach
work to connect with a greater diversity of local people & groups, who
may benefit from and/or wish to contribute to the wellbeing of local
community.
The job will involve accurate record keeping of patients,
maintaining clinical records and monitoring patient pathways.

includes Woodlands Road and Hawthorn Surgeries and Holmhurst Medical Practice
(all Redhill). The role is key to providing administrative support for
community health outreach projects.
The administrator will work closely with Practice teams and
community care teams to ensure patients receive appropriate care when it is
needed. The caseload will include proactive patient care as well as care
related to patients that have referred to the services.
Accurate record keeping, monitoring the patient pathway,

Attending meetings and reporting on progress to Practice
managers form part of the administration tasks for the role. The successful
candidate will demonstrate good teamworking skills as they will be part of both
