Wellbeing Care Coordinator

Job summary

We are looking for an enthusiastic and caring candidate for the post of Well-being Coordinator for Redhill Phoenix. The role requires great communication skills, ability to use ones own judgment, and initiative.

You will be required to work 10-12 hours per week, some of which can be remote but some will be in community settings.

Main duties of the job

The candidate will be required to work with the Redhill Phoenix PCN practices. They will support and establish community-led groups to improve their health and well-being, where a health need has been identified.

The candidate will work in collaboration with the GrowingHealth Together team and will spend time on outreach work to connect with a greater diversity of local people & groups, who may benefit from and/or wish to contribute to the wellbeing of local community.

The job will involve accurate record keeping of patients, maintaining clinical records and monitoring patient pathways.
