Job summary

This is an exciting opportunity to join a small, focused team of care coordinators supporting the organising and administration of care activities helping our patients live longer and happier lives at home.

You will be working closely with colleagues to ensure good use of clinical expertise focused on frail patients living their own homes and those resident in care homes. The administration of the annual health checks for patients with learning disability and those with severe mental illness also feature in the responsibilities of the role. There is also liaison with coordinators at East Surrey Hospital and other agencies involved in the care of patients in the community.

This role combines administrative work with direct patient contact to make the necessary arrangements to monitor long term conditions and ensure patients stay healthy and in control of their care. Integral to this are patient conversations on their care and recording of these in personalized care plans. Supporting weekly multidisciplinary team meetings and care home ward rounds complete the scope of work.

The role requires an enthusiasm for achieving task orientated goals and is best suited to someone who enjoys working with a constantly changing workload, covering multiple activities, where competing demands for their time can result in short deadlines to be met.

Main duties of the job

Ensuring clinical capacity is efficiently deployed by engaging with patients to ensure their long-term condition is managed with ease,

Maintaining long term condition registers,

Coordinating patient interventions with muti disciplinary teams,

Maintaining and updating patient cohorts,

Ensuring follow up patient activities are delivered,

Monitoring and collating information to report key performance measures,

Arranging patient appointments supporting Practice to deliver care,

Developing expert knowledge of the NHS primary care IT platforms and effectively using them to engage with patients to promote monitoring of their conditions.

Promoting the use of clinical templates to improve accuracy of information recording in patient clinical records,

Meet with practice GPs, other practice based clinical teams, community matrons, district nurses, voluntary sector organisations and wider health and social care services, to support patient care,

Keeping up to date records in GP clinical systems and service records,

Upload and maintaining patient care plans to clinical systems,

Teamworking to continually improve the care coordinator contribution to delivering personalised care.

The role works in Practices across the Redhill Phoenix Primary Care Network, which includes Woodlands Road and Hawthorn Surgeries and Holmhurst Medical Practice (all Redhill). The role is key to providing administrative support for PCN anticipatory care services that help our care homes patients and those living with frailty in the community as well as learning disability patients and those with severe mental illness. The administrator will work closely with Practice teams and community care teams to ensure patients receive appropriate care when it is needed. The caseload will include proactive patient care as well as care related to patients that have referred to the services.

Accurate record keeping of patients being seen by the anticipatory care services, maintaining clinical records, monitoring the patient pathway, collating key performance indicators, and arranging review appointments for patients with long term conditions feature in the workstreams of the coordinator.

Contacting individual patients by telephone, the post holder will ensure the necessary tasks have been completed to allow timely clinical reviews of conditions to take place. Referring to partner services for support with the patients care and ensuring the patient is at the centre of decisions about their care are essential to the success of our services; the candidate should be competent in liaising with multi-disciplinary teams.

There is the opportunity to help define service delivery as we seek to make the most of our available resources to work efficiently for the benefit of our patients.
