Job summary

Dorking Primary Care Network (PCN) is looking to recruit a Frailty Care Coordinator to work with the Community Medical Team to support our patients with frailty. The successful candidate will work across the four practices of Dorking PCN; Medwyn GP surgery, Brockwood Medical Practice, Leith Hill Practice, Capel and Dorking Medical Practice in this full-time role.

This is a hugely rewarding post and the Frailty Care Co-ordinator will have a pivotal administrative role in working with the practices and the wider primary/community care team to ensure frail patients receive appropriate interventions to support them to live as independently as possible. This will involve supporting the co-ordination of a range of clinicians and social care. It will also involve working with individual patients to identify, then facilitate any support they would benefit from being delivered by the voluntary sector or health services.

Main duties of the job

The ideal candidate will be an excellent communicator, organised, caring, conscientious and want to make a difference for our patients, by helping to plan proactive services for frail patients (planned care to support people to live independently). The post holder will work alongside the Community Medical Team and liaising with the Dorking GP practices.

If successful, you will play a key role in ensuring that healthcare professionals focus their limited resources on those in most need, co-ordinating the right intervention for each patient, ensuring consistent records are kept of care plans and interventions and ensuring that records are shared appropriately with all health and social care professionals involved in caring for each patient.

The PCNs are critical to the changing NHS and are the focal point of all locally developed planning and service delivery.

  • Educated to at least GCSE level or equivalent Proven experience of managing a complex administrative role Good understanding of the needs of the frail elderly Excellent administrative skills working under own initiative at times Strong, confident IT skills; able to use relevant Office packages (Word, Excel, Databases and Outlook for e-mail/calendar) and Databases (patient information). Comfortable working in often fast-moving and changing situations, with ability to adapt to the changing needs of the service Empathic, patient and caring; sensitive to peoples life stages, concerns and problems.
