Job summary

This is an exciting new opportunity to join the Enhanced Access team. This post is initially a 3 month fixed term position but may lead to a permanent position.

Role Summary:

To be responsible for a number of clinical areas in Women health such as SMEARS, health promotion, chronic disease management, health prevention, well women.

Job Purpose:

To act as an autonomous clinical practitioner working independently in conjunction with other healthcare professionals. Assess, diagnose and treat the conditions of patients attending primary care, initiating direct referrals as appropriate.

Provide expert professional advice to patients, carers and colleagues to ensure the maintenance of clinical excellence.

To develop new innovative concepts, models, methods and practices that support improved primary care services. By delivering on this meet the needs of the practice and BLMK population.

To provide education and training to other staff and students.

To undertake research as part of the role.

Main duties of the job

Key Responsibilities

Identify, manage, and support patients at risk of developing long-term conditions, preventing adverse effects on the patients health

Provide routine nursing care to patients as required in accordance with clinical based evidence, NICE and the NSF

Provide vaccinations and travel vaccinations

To be proficient in venepuncture

To provide medical review for Diabetes Clinics following the 8 step assessment

Provide wound care (Doppler) to patients

Provide chronic disease clinics, delivering patient care as necessary, referring patients to secondary / specialist care as required

Maintain chronic disease registers

Develop, implement, and embed well woman clinics

Implement vaccination programmes for adults and children

Recognise, assess, and refer patients presenting with mental health needs

Support the clinical team with all safeguarding matters

Deliver opportunistic health promotion where appropriate

Carry out smear testing and maintain smears results logs

Manage emergency drug kit

To adapt and develop the role of Advanced Clinical/Nurse Practitioner, ensuring flexibility and willingness to expand the role

Maintain accurate confidential health records

To provide leadership, education and training to different healthcare professionals, staff and students.

Provide autonomous care for the patients, to assess, investigate diagnose, treat and refer if necessary

To work at different locations if required

Understand practice and local policies for substance abuse and addictive behaviour, referring patients appropriately

Act as the audit lead, effectively utilising the audit cycle

Monitor and ensure the safe storage, rotation, and disposal of medicines

Support junior members of the nursing team, providing guidance when necessary

Participate in local initiatives to enhance service delivery and patient care

Support and participate in shared learning within the organisation.

Continually review clinical practices, responding to national policies and initiatives where appropriate

Enhanced Access Advanced Clinical Nursing Practice:

The ANP works autonomously and is accountable for their own professional actions and omissions.

Undertakes clinical nursing practice at an advanced level using expert knowledge and clinical skills to deliver holistic care to people accessing urgent primary care services.

Assesses diagnoses and treats patients attending Enhanced Access Clinics with a range of acute and chronic conditions.

To clearly communicate the health status of the patient using appropriate terminology, technology

Will provide a high standard of telephone/video triage and consultation as appropriate;
