Job summary
City and Hackney is advancing a bold neighbourhood-based
model of careanchored in Primary Care Networks (PCNs), shaped by communities,
and aligned with wider system reform across North East London. We are seeking a
senior clinical leader with a strong track record in system transformation to
guide the development of our eight integrated neighbourhoods.
This is a high-impact strategic leadership role offering the
opportunity to influence and embed a maturing neighbourhood infrastructure that
sits at the intersection of place-based partnerships, PCNs, and integrated care
priorities.
Main duties of the job
As Clinical Lead for Integrated Neighbourhoods, you will
provide strategic clinical leadership to ensure that neighbourhood activity is
both responsive to local needs and aligned with system-wide objectives. You
will work across organisational and professional boundaries to reframe local
delivery models, embed a shared population health approach, and strengthen the
enabling role neighbourhoods can play in supporting PCN and Place ambitions.
You will also play a key role in leadership development,
supporting the growth of a confident, skilled, and inclusive workforce to carry
the model forward.

- Provide clinical and strategic direction to ensure coherence and consistency across eight neighbourhoods
- Support PCNs to drive transformation by aligning neighbourhood priorities with operational and clinical goals
- Act as a senior system leader across borough forums, governance groups, and ICS transformation programmes
- Strengthen neighbourhood capabilities in data-driven planning, proactive care, and partnership working
- Facilitate cross-system collaboration between primary care, community services, the local authority, and VCSE partners
- Lead on clinical engagement, workforce development, and succession planning at neighbourhood level
What were looking for:
- GMC-registered clinician with extensive clinical leadership experience at system or borough level
- In-depth understanding of PCN development, MDT collaboration, and neighbourhood-based transformation
- Ability to operate with credibility across strategic forums and civic partnerships
- Skilled in coaching, organisational development, and change facilitation
- Committed to reducing inequalities and embedding population health management approaches
Familiarity with City and Hackney or comparable boroughs is desirable, as is experience using PHM tools and leading integrated initiatives.
