Job summary

The ARRS funding has created an opportunity for a GP who has qualified within the last 2 years to gain some experience working in our supportive and very friendly PCN practices in London Fields in Hackney.

Main duties of the job

We are a very highly performing and highly collaborative PCN. We would ensure a newly qualified GP has a full induction, mentoring with a senior GP, well-being blocks in their list, as well as pro-rata protected CPD and admin time weekly.

We are looking for up to 6 sessions or a job share of two GPs on 2-4 sessions each. If you are working across more than one site you will have a main host practice and a named GP at each site for advice and support.

In view of this being a short-term contract, we are keen to be flexible with sessions and will offer a highly competitive salary.

There is no set interview date, as the post would be available to start within a few weeks and the advert will close when a candidate is found.

London Fields PCN is a Primary Care Network (PCN) in Hackney East London, which is made up of six GP Practices. The PCN and Practice Teams consist of various roles, including advanced Practitioners, Clinical Pharmacists, Pharmacy Technicians, Physician Associates, Paramedics, Mental Health Pharmacists, First-Contact Physiotherapists, Social Prescribers, Health and Well-Being Coaches, Wellbeing Practitioners, and Care Coordinators, who serve a population of 56,000 patients.

As a new ARRS-funded role, the PCN is looking for a dynamic, enthusiastic and forward-thinking individual, who works well both independently and as part of the wider Multi-Disciplinary Team (MDT). The GP will be provided with a named supervisor throughout the post.

The funding for this role is currently guaranteed until March 2025, however, there may be possibilities to continue working with the Network beyond this date. In line with the funding requirements, the post holder must be a newly qualified GP (this is within two years of qualification and has not held a substantive post).

Job Purpose

The purpose of this role is to support the PCN in delivering high-quality care to the patients it serves. The role is intended to support the PCN and its practices in the delivery of their contract, which will require the post-holder to complete:

  • Face-to-face and telephone consultations
  • Home visits
  • Checking and signing repeat prescriptions
  • Administration - dealing with queries, patient test results, referral letters, tasks, emails and other paperwork and correspondence in a timely manner
  • Cover all the clinical management of the patient surgery as appropriate including initiating investigations, reviewing results, making referrals to secondary care or other providers as appropriate.
  • Develop care and treatment plans in consultation with patients and in line with current practice disease management protocols as well as wider guidance such as that from NICE.

Key Responsibilities

  • Providing safe, evidence-based, cost-effective, individualised patient care, making professional, autonomous decisions in relation to presenting problems.
  • Assessing the healthcare needs of patients with undifferentiated and undiagnosed problems. This includes clinically examining and assessing patient needs from a physiological and psychological perspective, planning clinical care accordingly and referring patients to other services/agencies in a timely manner, utilising relevant guidelines.
  • Supporting the delivery of advanced care plans.
  • Recognising the need for and being able to request and interpret investigatory procedures, including laboratory and radiology tests.
  • Screening patients for disease risk factors and early signs of illness and in line with disease management protocols.
  • Prescribing, in accordance with relevant local and national guidelines, where clinically appropriate.
  • Supporting patients to adopt health strategies that promote healthy lifestyles and apply principles of self-care, including providing counselling and supporting guidance on healthier living.
  • Ensuring appropriate follow up of patients.
  • Understanding the wider system support that is available to patients, and appropriately signposting / referring to wider Social Health or community organisation partners.
  • Recording clear and contemporaneous IT based consultation notes to agreed standards.

Organisational Expectations

The post-holder will be expected to undertake all normal duties and responsibilities associated with a GP working within primary care. They will be required to contribute to the medical administrative needs for the efficient running of the Practices and PCN, and attending training and events organised by the Practices and PCN. The post-holder will also be required to attend and contribute to MDT meetings across the PCN.
