Job Summary
a) Responsible
for providing primary medical services to the registered patients of the
practice, ensuring the highest standards of care for all, undertaken in line
with the relevant national primary care contractual regulations and guidelines.
b) Provide
care for both on-the day care for patients and pre-booked patients with long
term conditions.
c)
Responsible for maintaining the smooth running
of the practice.
3.
Key Responsibilities
All normal
duties and responsibilities associated with a Salaried GP working within
primary care as set by the regulatory and standard setting organisations,
including but not limited to the points below. In accordance with the practice
timetable, as agreed, will undertake a variety of duties including:
Surgery consultations and emergencies, telephone
consultations and queries, triaging, visiting patients at home, checking and
authorising repeat prescriptions and dealing with queries, patient test
results, patient medical reports and examinations on behalf of the Practice as
well as referral letters and other paperwork and correspondence in a timely
manner.
Provide health promotion and advice, signposting
patients to appropriate services to maximise their health and social
functioning and their ability to self-manage their care.
Be constantly mindful of the need for the
provision of personalised care and population health, including reducing health
inequalities and ensure that these are reflected in daily practice.
Make professional, autonomous decisions in
relation to presenting patients, whether self-referred or referred from other
health care workers within the organisation or external organisations.
Assess the health needs of patients with
undifferentiated and undiagnosed problems taking appropriate action as
necessary.
Screen patients for disease risk factors and
early signs of illness taking appropriate action as necessary.
Consult with patients and in line with current
disease management protocols and care pathways, develop care plans for health
related illnesses and share these in line with patient need and good
information governance.
Record clear and contemporaneous consultation
notes to agreed standards using IT systems.
Compile and issue computer-generated acute and
repeat prescriptions (avoiding hand-written prescriptions whenever possible) including
EPS.
Prescribe in accordance with locally agreed or
nationally agreed prescribing formulary.
Work in accordance with all relevant clinical
guidelines, pathways and protocols.
Responsible for tasks delegated to them,
appropriate to their level of competence and expected to delegate tasks or
responsibilities to other staff, as appropriate.
In-hours duty doctor sessions; to be agreed.
All General Practitioners are expected to
participate in continuing professional development, annual GP appraisal, Revalidation
and appropriate peer supervision and to remain up to date on professional
developments as required by their professional body. GPs are actively encouraged to take their
study leave entitlement in order to support the development needs identified in
their PDP and appraisal. Undertake all
requirements to remain on the Medical Performers List.
Ensure that he/she remains up to date with and
implements all relevant good clinical practice guidance (e.g. NICE guidance).
Work in accordance with all relevant clinical
pathways and protocols.
Active engagement with professional development
lead, to promote patient safety and quality of service.
Participation in a range of clinical governance
activities, including clinical audit and such quality improvement activities
deemed necessary by either Sussex Primary Care or external accrediting bodies.
Ensure that the requirements of the Care Quality
Commission are reflected in his/her day to day practice and will maintain and
develop the practices active management of CQC requirements.

Flexibility
a) This
job description is intended to provide a broad outline of the role. The
postholder may be required to carry out other duties commensurate with their
banding and competence.
6.
Policies and Procedures
a) The
postholder is required to familiarise themselves with all organisation policies
and procedures and to comply at all times.
7.
Confidentiality and Data Protection
a) The
postholder must maintain the confidentiality of information about patients,
staff and other health service business and always meet the requirements of the
Data Protection Act (1998).
b) The
postholder must always comply with all organisation information and data
protection policies. The work of Sussex Primary Care is of a confidential
nature and any information gained by the postholder in their role must not be
communicated to other persons except where required in the recognised course of
duty.
8.
Health and Safety
a) Employees
must be aware of the responsibilities placed on them under the Health and
Safety at Work Act (1974) and must follow these in full at all times, including
ensuring that they act in line with all agreed procedures at all times in order
to maintain a safe environment for patients, visitors and colleagues.
9.

a) Sussex

10.
Use of Technology
a) The
organisation is making increased use of computer technology. The majority of
employees (both clinical and non-clinical) should expect to use automated
information systems in their work in order to improve quality and co-ordination
of services, and to enable faster and more certain communication within the
Organisation. Necessary training will be provided.
11.
No Smoking Policy
a) Sussex
Primary Care operates a no-smoking policy, in line with Government
legislation. This applies to all staff,
visitors and patients. It is a condition
of employment for staff that they do not smoke whilst on duty or in uniform or
anywhere on Organisation premises. This includes electronic cigarettes.
b) Whilst
we do not discriminate against employing smokers, they are expected to adhere
to this policy and all prospective employees should be aware of this.
12.
Professional Registration
a) All
employees who are required to be a member of a professional body are required
to hold relevant registration and must comply with the standards and guidelines
of their professional practice, education and conduct and must act within the
boundary of the code of conduct at all times.
13.
Infection Prevention and Control
a) Infection
Prevention and Control is everybody's responsibility. All staff, both clinical and non-clinical are
required to adhere to the organisations Infection Prevention and Control
policy and make every effort to maintain high standards of infection prevention
and control at all times thereby reducing the burden of Health Care Associated
Infections.
14.
Safeguarding Children, Young People and
Vulnerable Adults
a) Sussex
Primary Careis committed to safeguarding and promoting the welfare of
children, young people and vulnerable adults and expects all staff and
volunteers to share this commitment. Rigorous recruitment checks are carried
out and successful applicants may be required to undertake an Enhanced
Disclosure via the Disclosure and Barring Service (DBS).
