Job summary
Change Grow Live are a large national Health and Social care charity. We exist to help people achieve their goals, to be the best that they can be and to help change society for the better. Each year we support thousands of people facing challenges in their lives including drugs and alcohol, housing, justice, health and wellbeing.
Changes to the Health and Social Care system and Drug Strategy developments are moving at pace. The purpose of the role is to lead on engagement and collaboration across the Health system to support collaborative working. You will be the link between the substance misuse service and the integrated care system, to join up care locally around peoples needs, reduce drug and alcohol harms and contribute to a reduction of health inequalities.
Building effective working relationships with ICBs including GPs, Pharmacies, Acute Trust and MH Trusts, your role will be pivotal in achieving improved health outcomes for people in Kirklees.
The role will deliver training and updates, collaborate on projects, develop pathways, manage contracts and raise the profile of the Specialist Drug and Alcohol treatment services within Kirklees.
Main duties of the job
This is a newly created role to build collaboration with our partners across the Integrated Care System. Youll need to understand the footprint and key priorities of system partners as well as utilise excellent relationship building skills to develop system wide working to address health inequalities. Ideally with a background in the NHS, youll bring an understanding of partners priorities, gaps and opportunities and work collaboratively within the healthcare system to promote joint working and the benefits that specialist substance misuse services can bring.
We offer hybrid working in our offices across Kirklees as well agreed time working at home. Attending meetings and delivering training to key partners in local Surgeries and the Trusts will be necessary to develop our existing relationships, so some travel within Kirklees is to be expected. However as with any newly created role, we will shape how this looks together as the role progresses.
