Job summary
As a Recovery Coordinator - Navigator, you will support individuals who have physical or
mental health barriers to accessing mainstream service.
You will support by developing a package of care via referrals to community
and statutory agencies and provide wraparound support to address physical
and mental health needs whilst delivering interventions to stabilise or reduce
substance use.
Main duties of the job
- Obtain referrals electronically and in person from third party sources as Hospitals, Mental Health Liaison Teams and GPs
- Liaise with hospitals as part of discharge planning to effectively support within the community
- Conduct Triage and assessments for all referrals from Hospitals and GP, as well as assessing referrals from GP surgeries and any self-referrals where mental or physical health barriers would impact on access to treatment
- Utilise home visits and community venues for one to one and group work sessions
- Make referrals to statutory and community organisation to address physical and mental health barriers
- Ensure a MDT approach to caseload management to ensure lead agencies are involved and actively supporting service users

- No formal qualification but have an excellent understanding of drug and alcohol issues and experience of working within a related field.
Desirable
- Advanced professional qualification in substance misuse, counselling, or social work.

- Have an understanding of the support and services available to address mental and physical health needs.
- To hold a driving license and have access to a car.
- Be an excellent communicator, both verbal and written with good IT skills
- Proactive and able to work on own initiative with excellent time management and prioritising skills.
- Have a good working knowledge of mental health interventions, services, and good practice.
- Be a supportive team player with strong interpersonal skills with the ability to work in partnership with a wide range of professionals, agencies, and internal/external stakeholders.
