We have an exciting opportunity for a Alcohol Shared Care Recovery Coordinator to work in local GP surgeriesas part of our busy Tower Hamlets service, delivering substance misuse interventions in community settings with a focus on Alcohol interventions.
Youll have to record information about what you do, so enjoying a bit of paperwork and attention to detail is important. But most of the time youll be enjoying working with people from all backgrounds, making a difference to their lives. No two days are the same and we treat every person who comes into our service as an individual, so being flexible, curious and a good listener are key.
Main duties of the job
Supporting service users from point of entry into the service and through their treatment/recovery journey
Work with the Shared Care GP to support service users to achieve sustainable recovery
Communicate with local GPSIs where applicable in relation to substance misuse treatment in Primary Care
Work in partnership with community Pharmacists, ensuring clear communication pathways in relation to supervised consumption arrangements, engagement/presentation and medication collection
Promote sexual health awareness, provide advice and information on protective factors and reducing risks and offer screening in association with the Practice Nurse
Provide screening, assessment, and recovery planning and onward referral
Reduce drug and alcohol related harm to service users and the wider community
Promote carer, service user and community involvement
Provide advocacy for access to partnership services
Work with service users to support social (re)integration, enabling them to lead meaningful and purposeful lives: promoting recovery, resilience, peer support and self-determination

About the role:
Work with the Shared Care GP to support service users to achieve sustainable recovery.
Carry out regular case reviews with the Shared Care GP and facilitate service user prescribing reviews with the GP (as directed by the GP.)
Communicate with local GPSIs where applicable in relation to substance misuse treatment in Primary Care.
Provide support to assist community detoxifications, working with GPs to ensure that service users have access to adequate resources, in line with CRIs detox protocols.
Work closely with other members of the primary care practice team as appropriate to ensure holistic healthcare needs of the service user are met.
n conjunction with nursing & medical staff support substitute prescribing regimes improving the wellbeing and meaningful activity of service users.
Working in partnership with community Pharmacists, ensuring clear communication pathways in relation to supervised consumption arrangements, engagement/presentation and medication collection.
Promote sexual health awareness, provide advice and information on protective factors and reducing risks and offer screening in association with the practice Nurse.
Offer advice and support to service users for BBV testing, and ensure appropriate signposting to additional support and services if needed.
Participate in regular supervision and maintain a current knowledge of appropriate interventions with regard to substance misuse.
Support annual Surgery Reviews, completing reports, templates and client surveys as needed/directed by CRI.
About you:
Have a strong understanding of substance misuse issues and experience of working within a similar field
Sound and demonstrable knowledge of clinical prescribing issues
Experience of community based support practices
Be an excellent communicator, both verbal and written with good IT skills
Proactive and able to work on own initiative with excellent time management and prioritising skills
Have a good working knowledge of mental health interventions, services, and good practice
Be a supportive team player with strong interpersonal skills with the ability to work in partnership with a wide range of professionals, agencies, and internal/external stakeholders
Ability to manage change successfully in a way that prioritises the needs of service users
Understand the importance of information governance processes and commit to follow and apply all necessary safeguards
Seek out learning opportunities to improve and broaden your professional knowledge and skills and to contribute and oversee the learning and development of others
