Job summary

North Hampshire Urgent Care, Virtual Care @ Home service is recruiting a contracted Nurse / AHP to work on the service. The Virtual Care @ Service currently cares for high - risk patients who are positive for COVID - 19 and suffering with Acute Respiratory Infection to support and monitor them at home and have developed a pathway to care for patients with COPD. Additionally, we support patients with exacerbations of Asthma and plan to increase the service to COPD and Diabetes.

This is an exciting time to join the service as we move to closer working with Hampshire Hospitals Foundation Trusts Virtual Wards, merging the services within the next financial year to create one service caring for the patients within North & Mid Hampshire. This will also facilitate an expansion of the service to other acute condition monitoring.

The post holder will oversee the Virtual Care @ Home service, accepting new admissions, using digital technology including InHealthcare and EMIS to ensure patients are contacted daily or at a clinically appropriate time. They will monitor the RAG-rated dashboard to identify any patient deterioration and act appropriately using advanced clinical skills.

This is a fixed term position from January - March 2024.

Main duties of the job

This role encompasses the skills of leadership and oversight of theVirtualCare@Home Service, using telephone triage to carry out assessment and monitoring within an agreed scope of practice. The role requires specialist knowledge across the range of work procedures and practices, underpinned by theoretical knowledge and relevant practical experience.

To work collaboratively with the Lead Nurse, Operations Manager other Nurses / Nurse Practitioners or ACPs. This may include covering each other, and in some cases working closely together and sharing a role.

To use advanced clinical skills to provide education to patients, promoting self-care, and empower them to make informed choices about their treatment. To participate in service development in conjunction with local colleagues and specialist practitioners as required

To prioritise health problems and intervene appropriately, including initiation of effective emergency care via the most appropriate services.

Promote health and well-being using health promotion, health education, screening and therapeutic communication skills.

To clearly communicate the health status of the patient using appropriate terminology and technology

To work collaboratively with the patients own GP & multidisciplinary team responsible for their care where required

The VirtualCare@Home service has been running since 2nd November 2020 and we have already managed and supported over 5000 patients with COVID on our COVID Oximetry@Home pathway.

The VirtualCare@Home Service currently cares for high- risk patients who are positive for COVID-19 and suffering with Acute Respiratory Infections to support and monitor them at home and has developed a pathway to care for patients with COPD. Additionally, we are supporting small number of patients with exacerbations of Asthma.

This is a rapidly expanding area of clinical care and the service is developing quickly alongside our colleagues in Hampshire Hospitals Foundation Trust, Southern Health Foundation Trust and Primary Care across North and Mid Place, alongside colleagues in HIOW ICS. We work closely as one Integrated Team in our area, providing seamless care for our patient population and over the coming year we plan to integrate fully with our colleagues at Hampshire Hospitals Foundation Trust.

The post holder will oversee the VirtualCare@Home service, accepting new admissions, using digital technology including InHealthcare to ensure patients are contacted daily or at a clinically appropriate time. The VirtualCare@Home Service will be run by North Hampshire Urgent Care working alongside a number of PCNs and working closely with Hampshire Hospitals Foundation Trust & Southern Health Foundation Trust. Governance Structures will be determined by North Hampshire Urgent Care. Within this structure, this role will be expected:

Administrative and professional responsibilities:

Day to day management of the VirtualCare@Home service including liaison with clinicians, advance planning and communication.

Participation in weekly MDT meeting / clinical huddle to discuss patients on the VirtualCare@Home service as required

Ensure accurate notes of all consultations and treatments are recorded in the patients notes on the clinical computer systems as appropriate.

Ensure accurate completion of all necessary documentation associated with patient health care including EMIS

Ensure collection and maintenance of statistical information required for regular and ad hoc reports.

Liaison:

Maintain effective liaison with other agencies and staff concerned with patient care and with all other disciplines with appropriate regard to confidentiality.
