initiative called Vital 5 which focuses on 5 key aspects of health, to give an
overall picture of a how healthy a person is & any areas they may wish to try
to improve. These include:
Healthy blood pressure
Stop smoking
Safe drinking
Healthy mind
Healthy weight
We are looking for compassionate and motivated people to
join the Vital 5 Health and Wellbeing Coaching service within Riverview PCN, as a Health &
Wellbeing Coach, in Greenwich, supporting residents living in the East and
Central East neigbourhoods (Thamesmead, Woolwich & Plumstead). This role will include carrying out the Vital
5 check with residents & then offering personalised,
one-to-one coaching to these residents, helping them identify meaningful goals
& overcome barriers to better health & wellbeing. The initial priority will be with residents
who have high blood pressure.
Working as part of a neighbourhood team, you will work
together with a number of GP practices within a defined geographical area to
support the residents they have identified.
You will also outreach into the local community (e.g. voluntary sector
organisations, faith groups, libraries) to support other residents who may
benefit from a Vital 5 check and health and wellbeing coaching. You will take a person-centred, holistic
approach to help individuals improve their physical, emotional and social
wellbeing.
Training will be provided for this role.
Main duties of the job
To carry out Vital 5 assessments with local
residents who are identified by their GP Practice or through the local
community (this can include people self-referring)
Deliver personalised, one-to-one coaching to residents
with identified, prioritising those with high blood pressure, helping them
identify meaningful goals and overcome barriers to better health and wellbeing.
Provide ongoing support to the resident for four
to six sessions, which could vary from follow-up phone calls or attending
appointments with the resident, centred around what is best for the resident.
Empower residents to take control of their own
health & wellbeing by ensuring they have access to appropriate support, are
aware of relevant services, and have a clear, person-centred exit strategy in
place prior to discharge.
Maintain accurate and timely records of coaching
sessions and outcomes using agreed systems and tools.
Actively feedback key themes and insight to
ensure continuing development and support of the development of integrated
neighbourhood working in Greenwich.
Support the maintenance and updating of the
database of local services and activities in Greenwich
Please note that the base of this role is within
Riverview PCN and you will be expected to travel across the PCN and work is
various locations as required.
Training will be provided for this role.

Safeguarding.
Identify, report, and monitor any safeguarding
risks and concerns.
Commit to undertaking any training considered
necessary to ensure and develop own knowledge and skills to effectively deliver
