Job summary

gtd healthcare is one of the largest NHS commissioned Primary Care, Out of Hours & Urgent Care providers in the North West of England.

We have a fantastic opportunity for a Nurse Associate to workwithin a community-based meeting of Multidisciplinary Team (MDT) Vascular/Diabetes clinic across the Oldham area.

This innovative service aims to deliver high-quality care in the community across five sites in Oldham, with the goal of reducing SDEC and A&E admissions for a specific patient population, namely those with vascular conditions and diabetes. The role will also involve providing complex care for diabetic patients and supporting ongoing management.

As a Nurse Associate, you will work as part of the nursing workforce to support registered nurses and other clinicians in delivering high-quality, safe, and compassionate care to patients

The successful candidate will join a small, close-knit team of dedicated clinicians and administrative staff, working collaboratively to deliver patient-centred care of the highest standard.

Main duties of the job

  • Work as part of the nursing workforce to support registered nurses and other clinicians in delivering high-quality, safe, and compassionate care to patients.
  • Provide evidence-based nursing care across the practice population, working collaboratively with a multidisciplinary team (MDT), in line with Nursing and Midwifery Council standards of proficiency and organisational policies and protocols.
  • Demonstrate the ability to work effectively within a team and provide support and guidance to less experienced staff and learners as required.
  • Act within professional boundaries while supporting and collaborating with the MDT within general practice and the Primary Care Network (PCN).
  • Contribute to the delivery of nursing care for patients referred to the service.
  • Take a lead role in specific areas, including vascular and diabetes care, ensuring compliance with CQC standards and promoting positive health outcomes and well-being for patients.
  • Work with MDT members to organise and facilitate care tailored to individual patient needs.
  • Uphold the services high infection prevention and control (IPC) standards through safe, consistent clinical practice.

You will be providing care to people of all ages, in a busy and diverse practice, contributing to core nursing roles and responsibilities, supporting the registered nurses to allow them to focus on more complex clinical care.

Following appropriate training, the following competency-based skills maybe undertaken by qualified nursing associates working within general practice:

  • Adult and child Immunisation under a Patient Specific Direction.
  • Live nasal flu vaccines under a Patient Specific Direction.
  • Travel vaccines under a Patient Specific Direction.
  • Shingles vaccine under a Patient Specific Direction.
  • Pneumococcal vaccine under a Patient Specific Direction.
  • B12 injections under a Patient Specific Direction.
  • Cervical Screening undertaking the role of cervical sample taker (in line with Public Health guidance on training requirements).
  • Taking of ECGs
  • Phlebotomy including home visits if required (the correct indemnity and car insurance will be required).
  • Monitoring of long-term conditions - QOF related reviews of complex patients.
  • Participate in clinical and non-clinical audit.
  • New patient health checks.
  • Spirometry.
  • Chaperoning duties as and when required.
