We have a fantastic opportunity for an experienced and motivated Advanced Clinical Practitioner to join our friendly and supportive primary care team at Hattersley Group Practice.
We are a busy but friendly Practice, dedicated to serving our registered patients across the Hyde community for over 25 years. The practice's are keen to develop and support staff to excel their career aspirations whilst making a positive difference to patients and the community.
As an Advanced Clinical Practitioner, you will work alongside a supportive multidisciplinary team who have a real commitment to continuous improvement, shared learning and best practice, while being passionate about providing patients with the highest possible quality care.
You will be joining a small, close-knit team of clinicians and administrative staff who work collaboratively to deliver safe, effective, and compassionate care to our patients. As a high QOF-achieving practice with excellent infection prevention and control standards.
We provide dedicated SPA time (4 hours per week, pro rata) to support professional growth and leadership development.
The hours of work for this post are 20 - 37.5 hours per week, Monday to Friday between to 08:00 and 18:30.
Main duties of the job
Work as part of the nursing and wider MDT to deliver high-quality, safe, person-centred and compassionate care to patients.

Provide advanced clinical assessment, diagnosis and management of patients presenting with a wide range of acute and chronic health conditions.

Work autonomously within your professional scope, delivering safe, evidence-based care in line with NMC standards, including the use of independent or supplementary prescribing where appropriate.

Support and collaborate with GPs, practice nurses, healthcare assistants, and other members of the Primary Care Network (PCN) to ensure effective and coordinated patient care.

Contribute to the delivery of QOF and CQC requirements through proactive management of long-term conditions, accurate record-keeping, and adherence to high clinical standards.

Lead and participate in quality improvement activities, including audits, service development, and implementation of best practice.

Provide leadership, support, and mentorship to colleagues and learners, helping to develop clinical skills and professional growth in the team.

Demonstrate effective teamworking, flexibility, and resilience in a dynamic primary care environment.
Ability to manage complex patients and workloads.

Maintain professional registration and continuing development in line with national frameworks for advanced practice and organisational policy.

Clinical Practice
- Assess and holistically clinically examine patients with both minor and complex needs through history taking, physical examination and appropriate use of investigations to confirm diagnosis.
- Plan, implement and evaluate an effective management plan for the patient, ensuring safe discharge/referral and/or transfer to an appropriate health care service.
- Provide information and advice on prescribed or over-the-counter medication on medication regimens, side-effects and interactions.
- Prioritise health problems and intervene appropriately to assist the patient in complex, urgent or emergency situations, including initiation of effective emergency care.
- Refer and discharge safely ensuring that safety-netting advice is offered.
- Triage telephone referrals, calls from external clinicians, patients and others into a range of services appropriately as required.
