We have an exciting opportunity for a Nurse Associate to
join our existing Enhanced Care Home Service (ECS). This is a full time post
working across two services within the ECS across the Manchester area.
The
Enhanced Health in Care Homes Service (EHCS) consist of 13 care homes situated
in Higher Blackley, Crumpsall and Harpurhey in North Manchester. The Discharge
to Assess (D2A - Pathway 3) Service care homes are situated in Droylsden and
Wythenshawe. The service runs Monday to Friday between 8am and 6:30pm.
Our Team constantly strives to deliver high quality
patient-centred care and the applicant will become a valued member of the team,
working alongside Advanced Clinical Practitioners, Senior Clinical
Practitioners, pharmacists, and other members of the wider Primary Care
Network.
As a registered Nursing Associate, you will play a key role
in the delivery of health promotion, phlebotomy, chronic disease management,
vaccinations, and other clinical roles in line with your scope of practice and
level of experience. You will need to be able to demonstrate that you can work
as part of a team, having both very good communication and organisational
skills.
Main duties of the job
The post holder will work as part of the nursing workforce supporting registered nurses and other clinicians in delivering high quality, person-centred and compassionate care to patients.
They will be part of the multi-disciplinary team delivering planned care to a group of patients working within the standards of proficiency outlined by the Nursing and Midwifery Council and in accordance with organisation's policies and protocols.
They will demonstrate the ability to work effectively as part of a team and provide support/guidance to less experienced staff/learners within the clinical environment as needed.
They are required to act within their professional boundaries, supporting and working alongside a multi-disciplinary team within general practice and the Primary Care Network (PCN).
The post holder will contribute to providing nursing care to patients registered in the GP practices.
The post holder will focus on assuming a lead role in specified areas, in line with the Quality and Outcome Framework (QOF), to support quality health outcomes, including health promotion and the well-being of our patients.
The post holder will work with other members of the Multidisciplinary Team (MDT) to both organise and facilitate care appropriate to the individual needs of the patients.

You will be providing care to people of all ages, in a busy and diverse practice, contributing to core nursing roles and responsibilities, supporting the registered nurses to allow them to focus on more complex clinical care.
Following appropriate training, the following competency-based skills maybe undertaken by qualified nursing associates working within general practice:
- Adult and child Immunisation under a Patient Specific Direction.
- Live nasal flu vaccines under a Patient Specific Direction.
- Travel vaccines under a Patient Specific Direction.
- Shingles vaccine under a Patient Specific Direction.
- Pneumococcal vaccine under a Patient Specific Direction.
- B12 injections.
- Cervical Screening undertaking the role of cervical sample taker (in line with Public Health guidance on training requirements).
- Taking of ECGs
- Phlebotomy including home visits if required (the correct indemnity and car insurance will be required).
- Monitoring of long-term conditions - QOF related reviews of complex patients.
- Participate in clinical and non-clinical audit.
- New patient health checks.
