Perform specialist assessment of patients needs, diagnose where appropriate, plan, implement and evaluate clinical treatment
Be able to discuss with patients and carers the range of possible treatment/ management options and their implications before treatment decisions are made.
Be responsible for monitoring health, safety and security of self, the team and others and promote best practice in the clinical area.
Identify clinical issues and incidents within the service that reduce the quality of care within the specialist service bringing any issues to the attention of the management team.
Ensure the delivery of safe and effective care andthat high standards of care are given and maintained and act when standards are not being met.
Physical Skills
Demonstrate competence in the use of all clinical equipment used in the overall care of patients within the scope of service.
Demonstrate competence in physical examination assessment skills applicable to scope of service.
Have competent computer / keyboard skills including ability to use EMIS
Main duties of the job
Perform assessment of patients needs, diagnose where appropriate, plan, implement and evaluate clinical treatment and care using advanced clinical knowledge and skills.
Establish effective communication with patients and carers / relatives.
Deliver specialised, evidence-based advice suitable for the individuals medical, social, psychological, religious and cultural requirements.
Ensure that appropriate action is taken to protect vulnerable adults and that they can access/receive the appropriate care.
Ensure compliance with policies, procedures and clinical guidelines for self and others.
Promote peoples equality, diversity and rights.
Identify clinical issues and incidents within the service that reduce the quality of care bringing any issues to the attention of the Clinical Lead.
Take part in activities to counter these to ensure the delivery of safe and effective care.
The post holder is responsible for ensuring the safety of patients at all times and promoting a safety culture through the effective management of risk.
Contribute and influence policy making and clinical guidelines internally and externally, necessary to support the service.

About us
Greenwich Health is the GP Federation for the Greenwich Borough. We are committed to providing quality to care to our patients whilst building resilience for General Practice and delivering services and programmes of work to deliver care at scale to the Greenwich Population. The post holder will be required to support a range of different services the organisation provides. This will largely be between working with our GP Extended Access services
GP Extended Access Hubs Our service operates from two sites across the borough and provides GP services to Greenwich residents 5 days a week.
Principal duties and responsibilities
Take responsibility for the patient list for any assigned sessions within our Enhanced Access Hubs
Perform assessment of patients needs, plan, implement and evaluate clinical treatment and care using advanced clinical knowledge and skills.
Be able to discuss with patients and carers the range of possible treatment/ management options and their implications before treatment decisions are made.
Instigate and implement clinical management and treatment changes when appropriate.
Contribute to specific care guidelines and standards supporting the care of patients across the health economy.
To implement Health and Safety and Risk Management appropriate to area of work
To deliver aspects of the Clinical Governance Agenda, following the direction of the Clinical Leadership.
