This role is for the Beeston, Middleton and Hunslet Primary Care Network (PCN)
Primary Care Networks (PCNs) form a key building block of the NHS long-term plan. Bringing general practices together to work at scale has been a policy priority for some years for a range of reasons, including improving the ability of practices to recruit and retain staff; to manage financial and estates pressures; to provide a wider range of services to patients and to integrate with the wider health and care system more easily. PCNs were formally ratified in July 2019.

Beeston, Middleton & Hunslet PCN is seeking a Senior Occupational Therapist to be part of our multi-disciplinary and multi-agency team of professionals. We are looking for an individual who is passionate about Primary Care and is keen to work across traditional health and social care boundaries. This is open to anyone with relevant registration. This is an exciting and unique opportunity to be at the forefront of the service delivery of our new PCN Team, including Proactive Care Team. Working both independently, and within our multi agency team, the role will operate across the traditional health and social care organisational boundaries. This includes our GP practices and will deliver enhanced proactive care for older people with frailty.We are looking for an Senior Occupational Therapist with a strong interest in working in Primary Care and who aspires to provide a continuous improvement approach to delivering responsive, safe, patient-centred, and effective care within a team. The service, within which the post holder would work, supports patients with multifaceted health problems living in their own homes. The service liaises with carers, families, and colleagues to maintain and maximise individualised patient-centred care. It also supports General Practice in delivering exciting new developments and this can be flexible depending on skills, expertise, and interests.

Beeston, Middleton and Hunslet PCN (BMH PCN) work collaboratively together, this role is designed to work across both, ensuing a continuity of care between. BMH PCN has a patient list size of circa 82,000. We have a significant workforce in place which brings wonderful opportunities for in-house multi-disciplinary team working. We have a well-developed multidisciplinary team including a Pharmacy Team, Occupational Therapists, Social Prescribers, First Contact Physiotherapists, a range of nursing/ HCA staff and more which means that the majority of any patient's needs can now be provided on their own doorstep.

Main Tasks:
Clinical
Deliver expert knowledge, advanced clinical reasoning and care planning skills to the multi-disciplinary team.
To use advanced knowledge and practical experience to supervise, co-ordinate and manage the proactive care planning of the designated patient cohorts
To liaise with other health and social care professionals and agencies to ensure holistic care is provided and the care packages offered are appropriate and consistent with the patients needs and aspirations.
To ensure that the patients and their families and carers are fully involved in the assessment and care planning process.
To ensure best practice relating to care planning and the management of frailty and long-term conditions is adhered to
Managerial Responsibilities:
To co-ordinate, monitor and assess the work of the PCN Proactive Care Team
Provide Clinical Supervision to the PCN Proactive Care Team on a regular basis
To provide emotional support for team members in potentially stressful work situations or adverse incidents
To identify any service improvements or efficiencies
Be involved in service development
To contribute to the staff recruitment process
To complete return to work interviews with staff following sickness in line with the Sickness Absence policy
