Job summary

Middleton & Hunslet PCN is seeking an Advanced Clinical Practitioner (ACP) to join our multi-disciplinary and multi-agency team of professionals for one day per week (7.5 hours per week). We are looking for an individual who is passionate about Primary Care. This is open to anyone with relevant registration. This is an exciting and unique opportunity to be at the forefront of the service delivery of our new PCN Team, including the Frailty Hub. Working both independently, and within our multi agency team, the role will operate across the traditional health and social care organisational boundaries. This includes our GP practices and will deliver care into care homes and enhanced proactive care for older people with frailty. We are looking for an Advanced Clinical Practitioner with a strong interest in working in Primary Care and who aspires to provide a continuous improvement approach to delivering responsive, safe, patient-centred, and effective care within a team. The service, within which the post holder would work, supports patients with multifaceted health problems living in their own homes as well as in care homes. The service liaises with care home staff, carers, families, and colleagues to maintain and maximise individualised patient-centred care (including advanced care planning). It also supports General Practice in delivering exciting new developments and this can be flexible depending on skills, expertise, and interests.

About us

Primary Care Networks (PCNs) form a key building block of the NHS long-term plan. Bringing general practices together to work at scale has been a policy priority for some years for a range of reasons, including improving the ability of practices to recruit and retain staff; to manage financial and estates pressures; to provide a wider range of services to patients and to integrate with the wider health and care system more easily. PCNs were formally ratified in July 2019.Middleton and Hunslet PCN is a large PCN with a patient list size of circa 33,000 patients. Of this, we currently provide clinical support to 10 Care Homes, with a population of around 360 beds. We have a significant workforce in place which brings wonderful opportunities for in-house multi-disciplinary team working. We have a well-developed multidisciplinary team including a Pharmacy Team, Occupational Therapists, Social Prescribers, a range of nursing/ HCA staff and more which means that the majority of any patient's needs can now be provided on their own doorstep.Middleton & Hunslet PCN also works collaboratively with Beeston PCN, forming BMH (Beeston, Middleton & Hunslet PCNs, we have roles working across both ensuring a continuity of care between. To note this role is for that only of Middleton and Hunslet PCN.

Essential

  • Registered Nurse
  • NMC 1st level Registration
  • Educated to Masters Level Nurse
  • Independent Prescribing Qualification
  • Evidence of continued professional development
  • Able to demonstrate on-going personal and professional
  • development relevant to role.
  • Excellent communication skills including verbal, written, face to face and active listening skills.
  • Able to discuss difficult and/ or complex matters sensitively and diplomatically with patients and families, including End of Life/
  • Advanced Care Planning.
  • Recognise peoples needs for alternative methods of communication and respond accordingly
  • Able to demonstrate experience in conflict management
  • Computer skills; Microsoft Word and e-mail
  • Ability to initiate, sustain and evaluate change
  • Knowledge and awareness of current local and national NHS policy
  • Is able to demonstrate skills in problem solving and decision making at advanced practitioner level
  • Commitment to quality improvement principles
