Do you have the skills, experience, and desire to
support and influence the health and wellbeing of patients to tackle health
inequalities. We our looking for a Nursing Associate to do that role as part of
a Primary Care Network (PCN) multidisciplinary team. The post holder will mainly
focus on supporting patients with multimorbiditys like diabetes, chronic
kidney disease, serious mental illness and learning disabilities. They will be
responsible for ensuring the effective holistic provision of high-quality
nursing care to people in clinics at community events and in outreach
environments. There is a mixture of clinical work and as well great
opportunities to work with patients in group learning sessions and engage with
the community at arranged events and group sessions.
Main duties of the job
This exciting role with see the Nursing Associates
actively contribute to a patients health and wellbeing through the delivery of
holistic care, a whole-person approach which considers, and equally values,
physical, psychological, public health needs, social economic, spiritual and
other factors in the assessing, planning and delivery of care. Providing all
relevant competencies (to be developed in line with the requirements of the
service) have been achieved, she/he/they will have delegated responsibility for
a pre-agreed caseload under the supervision of a nominated clinician within the
PCN multidisciplinary. The post holder will be integral to the team of clinical
pharmacists, specialist Mental Health HCA, care coordinators, care home
patients and a womens health hub, GPs, Nurses, Health and Wellbeing Coaches,
Social prescribers and the wider local care partners like: Community services,
hospitals, specialist for long term conditions along the patients pathway.

health and Learning disabilities (multimorbidity) is very common and is a known
priority for BHR PCN. We think this is a
challenging but very rewarding opportunity for a nursing associate to join our
commited and dedicated team that tackle health inequalities daily. Our
population have a higher prevalence of mixed mental and long-term conditions
because of the nature of our more deprived communities, particularly at an
earlier age. Along with our culturally diverse population, these multimorbidity
patients are typically our hard-to-reach patients.
Salary is dependent on NHS experience and current
banding/pay point, applicants from outside the NHS will receive the start point
of the salary scale in line with NHS terms & conditions. Benefits package
