Park PCN has a patient population of approximately 100,000 across 10 practices. We aim to provide high quality services
adhering to principles of best practice, promoting equal opportunities and working

As a Health and
Wellbeing Coach you will work as a key part of the primary care network (PCN)
multi-disciplinary team. You will consider the whole person in addressing
existing issues and encouraging the proactive prevention of new illnesses,
promoting lifestyle medicine approaches.
Health coaching
revolves around using your coaching skills to support people with lower levels
of patient activation to develop the knowledge, skills, and confidence to
manage their health and wellbeing, whilst increasing their ability to access
and utilise community support offers.
Main duties of the job
The post holders will work in partnership with their clinical and non-clinical colleagues, management support and the wider PCN to ensure the role delivers the best possible outcomes for our patients.
The role is patient facing, varied, rewarding and sometimes challenging and may include supporting self-management education, peer support, case management and facilitating group consultations. You will take an approach that is non-judgemental, based on strong communication and negotiation skills. You will support personal choice and positive risk taking, while ensuring that patients understand the accountability of their own actions and decisions.
As a whole your role
and skills will support and encourage the prevention of developing further
illness, or the deterioration of existing long term conditions.

Woodsley & Holt Pak PCN covers the following areas: Woodhouse, Kirkstall, Burley, Hyde Park, Headingley, Cookridge and Horsforth areas of Leeds, although it is expected the post holders will develop and maintain close links across the city.

- Good technical literacy of Microsoft Applications e.g. Word, Excel, and Access etc.
- Good interpersonal skills including listening and displaying empathy

- Understanding of the barriers people face to accessing services and how to overcome them
- Ability to support and motivate people to make sustained changes in their lives
- Ability to reflect on and share practice with peers
- Have access to a car and being in possession of a full current driving license and business insurance
