ANP Job Description

Salary: £50,952 -£57, 349 (dependent on experience)

We are looking for enthusiastic, committed and professionally registered candidates with the clinical expertise and confidence to assess and treat patients face to face with a range of acute conditions.

Main duties of the job

This is an exciting new role, within a Primary Care setting, working as part of the Woodsley and Holt Park Primary Care Networks (PCNs), where you will be using your skills to support and compliment our wider PCN Multi-Disciplinary Team.

You will be a Registered Advanced Nurse Practitioner who will work autonomously providing high quality and timely patient care. You will be an independent prescriber who is motivated, proactive, committed and with excellent communication skills. You will work under the leadership of our Clinical Director, supporting with and improving on the safety and quality of all aspects of patient care at every opportunity.

About us

Our Team

The Woodsley and Holt PCN formed in 2019 and consists of 10 GP Surgeries; High Field, Ireland Wood Surgery, Abby Grange, Burley Park, Burton Croft, Craven Road, Hyde Park,, Kirkstall Lane, Laurel Bank, Vesper Road.

We cover a wide and varied 116,140 Patient population. We have developed a successful workforce compromising 49 staff members all lead by our Clinical Director and Leadership Team.

All our services are growing and embedding within our member practices. We now wish to add an ANP service to the team and believe this will be instrumental in continuing to meet the challenges in Primary Care and provide high quality and responsive treatment to all our patients with a focus on same day acute access.

Job Responsibilities

Key responsibilities, in particular but not limited to, are;

Act as an expert practitioner, demonstrating advanced clinical competence and a knowledge base beyond those associated with conventional nursing roles.

work as an autonomous clinical practitioner/nurse providing high quality and timely patient centred care.

Be an independent prescriber, working within the current guidelines, reviewing, and monitoring patient medications in accordance with the formulary.

Have experience of delivering face to face/telephone/e-consult/video consultations as well as home visits. Directing care appropriately and managing complete episodes of care independently.

Undertake consultations with patients of all ages including children as an autonomous practitioner and using own clinical judgement, diagnose, treat, refer and/or discharge patients.

Work within and across the MDT teams and signpost/ draw upon expertise of all members to support and meet the patients best needs and interests.

Record clear and contemporaneous consultation notes to agreed standards, ensuring accurate read coding.

Participate in and contribute to all clinical targets (QOF, QIS, ES etc)

Recommend, instigate, and explain appropriate invasive and non-invasive diagnostic tests, investigations and interpret findings and reports.
