Cross Gates Primary Care Network includes four practices working across seven sites: Ashfield and The Grange Medical Centre, Colton Mill and The Grange Medical Centre, Family Doctors and Manston and Surgery. The PCN has a patient population of 30,000 including patients living across 5 care homes.

We believe in providing a holistic approach to managing patient care and supporting individuals to meet their own needs and aspirations. You will be part of an organisation whose leadership team are supportive and innovative, focused on change and transforming services.

Our team currently includes Clinical pharmacists, pharmacy technician, mental health practitioners, advanced clinical practitioner, admiral nurse, nurse associate, physician associate, first contact physiotherapists and care coordinators, and aims to support our practices and improve health outcomes for our patients. We do this through the integration of PCN teams and services with our practices, continuous evaluation, and ongoing development of our services and projects, looking for opportunities for innovation and transformation and sharing best practice.

In addition, we have built close working relationships with statutory and voluntary sector organisations including Linking Leeds, Memory Support Workers, LCH Neighbourhood teams, Cross Gates Good Neighbours, our Local Care Partnership, community pharmacies and our care homes.

Main duties of the job

Job Summary

You will work within the PCN multidisciplinary team and be expected to undertake acute and planned visits to patients in their own homes and those living in residential and nursing homes and be able to appropriately assess, examine, investigate, diagnose and treat a wide range of complex or urgent conditions, resulting in the safe management and appropriate referral of patients with undifferentiated, undiagnosed and complex presentations. You will also support practices with on the day demand by delivering acute clinics for patients across all four practices.

You will also be expected to take on a leadership role within the PCN clinical team providing the team with clinical supervision and supporting the team in their day-to-day responsibilities across the PCN. Working with the Clinical Director, PCN Manager and clinical leads you will ensure that all the PCN employed staff are working to agreed protocols and are delivering services in line with the PCN plans.

You will be expected to work flexibly outside core hours where required and be able to adapt to changes within primary care when needed.

The post has two main functions:

  • To carry out the duties as outlined and
  • To be proactive in continuously improving service delivery.

About us

The Crossgates PCN population is generally slightly older than the Leeds average and has far fewer adults under 45. Patients live in all areas of Leeds and the majority are in the second least deprived fifth of the city. GP recorded ethnicity shows the BAME group to be much smaller than in Leeds overall. Most health conditions are average and following Leeds trends, obesity is significantly higher than Leeds though and also increasing more quickly. Frailty rates are average or better than Leeds, while under 75 mortality rates are almost identical to the city. Despite this life expectancy is significantly below averages and on a par with more deprived PCNs

Clinical Responsibilities:

To assess and triage patients, including same day triage, and as appropriate provide definitive treatment (including prescribing medications following policy, patient group directives, NICE (national) and local clinical guidelines and local care pathways) or make necessary referrals to other members of the primary care team for patients unable to attend the surgery and requiring a home visit or to those patients living in residential or care homes within the PCN area.

To work at an advanced level of practice working within PCN policies and procedures and your own professional codes of conduct

You will assess, examine, investigate, diagnose and treat patients in practice, their own homes or in a care home setting undertaking relevant history taking and physical examinations managing undifferentiated undiagnosed conditions and identify red flags and underlying serious pathology and take appropriate action.

Demonstrate the ability to make sound clinical decisions supported by complex decision making within a broad scope of practice using evidence-based practice to inform your decisions.

Manage a range of patient long-term conditions including but not limited to Asthma, COPD, Diabetes and CHD following practice guidelines and supporting the practice in achieving management of these conditions in line with QOF, QIS and other local guidelines and targets.

Actively take a personalised care approach prioritising health needs and creating clinically appropriate treatment and management plans, ensuring the patients views are considered in the decision-making process.

Identify the need for appropriate diagnostic tests and demonstrate the ability to interpret results.

Prescribe and review medication within relevant professional scope of practice and training appropriate to patient needs and in accordance with evidence-based practice, national and local protocols, and within scope of PCN.

Ensure patients are informed and consent is obtained prior to treatment, investigation and management, as appropriate.

Provide health promotion advice.

Appropriately refer patients to the wider MDT to ensure the safe management and treatment of the patient cohort.

Continuously evaluate the patients condition and response to therapeutic intervention and treatment modalities, modifying the care plan for optimal patient centred outcomes, responding in a timely manner

To maintain up-to-date contemporaneous clinical records in accordance with national guidelines.

To share knowledge and expertise, acting as an expert resource to others across the PCN, as appropriate.

Consult by telephone, online and face to face as appropriate.

Working with the PCN manager to ensure that all PCN employed clinical staff are appropriately supported in their day-to-day roles and can work to agreed job plans and are delivering services that meet the PCN DES and other plans.

Accountable for own professional actions, undertaking all duties in accordance with Codes of Professional Conduct, PCN policies and protocols that apply to the work you are undertaking.

To identify and intervene proactively where circumstances contribute to an unsafe environment for patients, staff or relatives and escalate these appropriately.

Actively implement effective systems and processes for infection prevention and control management and relevant audit within their clinical area

Lead and develop advanced clinical practice, developing and implementing protocols and procedures for emergency care that impact across the PCN.

Contribute to the review and development of clinical patient pathways to ensure a standardised approach to care.

Adapt specialist clinical knowledge and skills to different clinical settings and influence service delivery and patient care.

Work with your line manager to access regular clinical supervision, to enable you to deal effectively with the difficult issues that people present.

Promote and adhere to the principles and practice of clinical governance.

To maintain knowledge and training in key areas to support professional competence including safeguarding, infection control and keep mandatory training up to date.
