We are looking for forward thinking, motivated Health Check Assistants who have a passion for new ways of working whilst providing safe patient and effective care.

Job Type: Bank Staff hours flexible (annual leave/sickness cover 2 shifts per month minimum)

Operates: The service operates 7 days a week, working hours vary day to day

Pay: £12.45 per hour with enhancements for weekend and bank holidays

Main duties of the job

The successful candidate will work providing health checks and offering lifestyle advise to patients across Lancashire. Observations include SP02 levels, heights, weights pulse rates, BP checks, point of care testing for cholesterol and pre diabetic screening where appropriate. This information will be inputted in real time to the EMIS IT system including calculation of the QRisk3 score.

Full training for point of care testing will be provided and other systems where required.

Job responsibilities

Role of the Service

As part of our diagnostic business unit, we are working across various locations in Lancashire to provide NHS Health Checks to eligible patients aged 40-74. As part of this check, patients have observations taken as well as Point of Care testing for cholesterol and when appropriate HBA1c levels are checked. For those not eligible, patients are offered a mini health check for observations. All patients are given lifestyle advice and when appropriate signposted to other services. These health checks can take place in various locations including on our mobile treatment unit, local community venues, shopping centres and workplaces.

Duties and Responsibilities

  • To undertake NHS health checks in the community face to face with patients.
  • To provide patient education and health promotion.
  • Work to attain the service KPIs and targets.
  • Ensure all patient contact is recorded in a timely manner on the IT systems to enable tracking of patient outcomes, both in real-time to support ongoing intervention planning and retrospectively to understand the impact of the service.
  • To ensure mandatory training is kept up to date.
  • Acknowledge and accommodate barriers to communication and understanding including speech, hearing, language, and emotion.
  • Flexibility in attending organised educational updates and eagerness to expand on current knowledge base and cascading new educational information to colleagues whenever possible
  • Inform the Clinical Manager of any untoward incidents, work within all relevant policies and procedural guidelines e.g. infection control, safeguarding and risk management.

Education and In Service Training

  • Be fully conversant in all the clinical computer systems used within FCMS.
  • Attend relevant study/induction days, seminars, courses etc, for individual development and for the benefit of the service.
  • Where necessary relevant training in the operation of new or unfamiliar equipment, software or procedures will be provided or arranged.

Confidentiality

In the course of your duties, you may have access to confidential information about patients, staff or health service business. On no account must such information be divulged to anyone who is not authorised to receive it. Confidentiality of information must be always preserved whether at or away from work. FCMS has in place a Whistle blowers Policy for staff wishing to express concerns.

Data Protection Act, 1998

Carry out any requirements within the duties applicable to the Data Protection Act, 1998.

Health and Safety At Work Act, 1974
