Job summary

An exciting opportunity for a clinically strong and highly motivated Clinical Pharmacist to join our friendly and helpful team at Mayfield Medical Centre. We have a multidisciplinary team consisting of advanced health professionals including, Paramedics, First Contact Physiotherapists, Health Care Assistants, Practice Nurses, Nursing Associate, Mental Health Workers and Salaried GPs.

Salus Medical Services hold the contract for Mayfield Medical Centre, which is a new and exciting change to how practices are run. Rather than having a Partnership to contend with, as a non-for-profit company, all the money the practice earns is reinvested back into the practice. We facilitate an innovative and friendly atmosphere, where ideas are encouraged, everyone is welcome and everyone works as part of a team. Our ultimate vision is to work within the local community to offer preventative style healthcare. The practice is located in north Farnborough and has a list size of 9500 patients. We would be very excited to welcome a new pharmacist as part of this exciting new vision.

Main duties of the job

The practice is looking to recruit a competent prescriber and who can assist with the below:

Working as part of a multi-disciplinary team in a patient facing role to clinically assess and treat patients using their expert knowledge of medicines for specific disease areas

Supporting patients and the practice(s) with chronic disease management

Undertaking clinical medication reviews

Managing prescription processes and medication queries

Providing clinical leadership on medicines optimisation and quality improvement (including QOF) and CQC

Promoting collaborative working between general practice, our local acute trust and community service providers.

About us

Salus is a GP Federation which was formed by the local GP practices within North East Hampshire and Farnham. This Includes practices located in Yateley, Farnborough, Fleet, Aldershot & Farnham.

We work on behalf of local General Practice to provide services at scale to our member practices.

We have a very successful team of Paramedic Practitioners working to provide home visiting services, on the day clinics in practices and nursing homes.

We work at the centre of Integrated Care locally with our own team of nursing, clinical associate, care- co-ordinating and administrative staff.

We act as an employment vehicle for ARRS staff working in local PCNs/ Practices.

We have an Investors in People Silver Award and strongly believe in training and support helping our staff develop to reach their full potential and achieve fulfilling careers.

We were part of the first wave pilot scheme welcoming pharmacists to come and work in general practice back in 2016 and have employed pharmacists ever since.

The senior management team at Salus Medical Services have a wealth of experience working in general practice and therefore a good understanding of the opportunities and challenges faced.

Since 2019 we have held the APMS contract for the running of Mayfield Practice in Farnborough.
