Mayfield Medical Centre are building a new model for primary care with no Partners - we are all salaried and have a fantastic team of allied health professionals to work with. It is an interesting practice population and now is a great time to join while we are still working out how best to provide good primary care in a way of working that is manageable.
To work as part of the clinical team dealing with a wide range of health needs in a primary care setting, ensuring the highest standards of care for all patients whilst building effective working relationships with other health care professionals.
We can offer:
- Flexible 3-7 sessions, days and times negotiable

Main duties of the job
- In accordance with the practice timetable, as agreed, undertake a variety of duties, including surgery consultations, telephone consultations and queries, checking and signing repeat prescriptions and dealing with queries, paperwork and correspondence in a timely fashion. Actioning a share of blood results and incoming clinical documents, both paper and electronic
- Making professional, autonomous decisions in relation to presenting problems, whether self-referred or referred from other health care workers within the organisation
- Assessing the health care needs of patients with undifferentiated and undiagnosed problems
- Screening patients for disease risk factors and early signs of illness
- Developing care plans for health in consultation with patients and in line with current practice disease management protocols
- Recording clear and detailed consultation notes to agreed standards
- Prescribing in accordance with the practice prescribing formulary (or generically) whenever this is clinically appropriate.
- Ensure when inputting to a patients record that the correct templates are used and the relevant information is coded.
- In general, the post-holder will be expected to undertake all the normal duties and responsibilities associated with a GP working within primary care.

Mayfield
Medical Centre is a GP Surgery run by Salus Medical Services.
Salus
is a GP Federation which was formed by the local GP practices within North
East Hampshire and Farnham. This
Includes practices located in Aldershot, Farnborough, Fleet, Farnham &
Yateley. We hold several contracts
supporting primary care to deliver services on behalf of those practices,
helping them work better together and with other healthcare providers leading
to improved patient services and developing new ones due to constant changes
in the Primary Care environment.
We
also assist practices with admin, operational and HR support and facilitate
shared learning across localities. By
engaging with the public, we also help ensure that services are designed and
developed with patient and public involvement.
The
staff at Salus Medical Services have a wealth of experience working in
general practice and therefore a good understanding of the opportunities and
challenges faced.

- 6 Doctor Practice (9,500 patients), well supported by a strong multi-disciplinary team
Daily Paramedic Practitioner support (reserved for GPs to book appropriate urgent on the day appointments
Paramedic Led Triage twice per week this has proved a very popular way of working. Essentially means that the duty GP that session will only see patients which are not appropriate for a Paramedic Practitioner.
- Home visits undertaken by Paramedic Practitioners.
- Direct Access Mental Health Support Worker on site 5 days per week.
- Full time Clinical Pharmacist.
- Practice-based Paramedic Practitioner.
- Direct access First Contact Physiotherapist.
- Modern, purpose-built premises.
- Strong links with Farnborough Primary Care Network and particularly good relationships with other local practices and the local place ICB primary care team.
- North East Hampshire was a Vanguard Site.
- CQC good rating in all domains.
- Consistently high achieving QOF practice.
