Job summary

The Durham Dales Health Federation is a formal collaboration of the 12 GP Practices in the Durham Dales locality.The Pharmacy Technician will complement Clinical Pharmacists, community pharmacists and other members of the Primary Care Network multi-disciplinary team.The Pharmacy Technicians core responsibilities will cover clinical, and technical and administrative duties.Working under the supervision of the Senior Clinical Pharmacist.The post holder will ensure effective and efficient use of medicines. This will involve a targeted medication review, supporting patients and carers post-hospital discharge, health promotion, and support for self-care.

Main duties of the job

Undertake patient facing & supporting roles to ensure effective medicines use, through shared decision-making.Carrying out medicines optimisation including effective medicine administration, supporting medication reviews and reconciliation. Utilise consultation skills to work in partnership with patients to ensure they use their medicines effectively. Supporting medication reviews and medicines reconciliation for new care home patients and synchronising medicines for patient transfers between care settings, linking with local community pharmacists, and referring to the pharmacist for structured medication reviews.Providing specialist expertise to address local health inequalities.Taking a central role in the clinical aspects of shared care protocols and liaising with specialist pharmacists for more complex patients. Help to reduce inappropriate antibiotic prescribing.

A group of 12 GP practices across Durham Dales got together, looking for a solution to help them provide the care patients needed, share resources and ideas, and be prepared for the challenges the NHS would face in the future.The answer they came up with was a formal collaboration between the dozen practices: the Durham Dales Health Federation. DDHFs founding principal is to work together to provide high-quality, cost effective primary healthcare services.That means providing a range of services that all GP practices in the area can use to help look after patients.We employ a range of clinical staff pharmacists, dispensers, nurses, emergency care practitioners and social prescribing link workers as well as non-clinical, administration support staff.

Working with the PCN multi-disciplinary team to ensure efficient medicines optimisation, including implementing efficient ordering and return processes and reducing wastage.Providing training and support on the legal, safe and secure handling of medicines, including the implementation of the Electronic Prescription Service (EPS).Developing relationships with other pharmacy technicians, pharmacists and members of the multi-disciplinary team to support integration of the pharmacy team across health and social care including primary care, community pharmacy, secondary care and mental health.Supervising practice reception teams in sorting and streaming general prescription requests, so as to allow GPs and clinical pharmacists to review the more clinically complex requests.Supporting the implementation of national prescribing policies and guidance within GP practices, care homes and other primary care settings. This will be achieved through undertaking clinical audits (e.g. use of antibiotics), supporting quality improvement measures and contributing to the Quality and Outcomes Framework and enhanced services.   • NVQ Level 3 Diploma or equivalent in the Principles and Practice for Pharmacy Technicians or similar
