Main duties of the job

Responsibilities will include:

  • Assessing and Reviewing the Clinical needs of our Children and maintaining effective Care & Risk Planning
  • Ensuring the service we are providing is meeting the needs of our families, including direct clinical support
  • Supporting the Consultants with the recruitment, retention and supervision of the team, getting the right people for the right families
  • Management of Complex Clinical Needs and Medication Management
  • Participating with the Reviewing and Enhancing Company Policies
  • Training and Mentoring staff with Clinical Interventions including Competency Assessments
  • Supporting the Business with a On-Call advisory support on a fair rotational basis
  • Auditing and Improvement Planning

Job responsibilities

You will co-ordinate referrals including identifying client service requirements and initial comprehensive care assessments to formulate care plans, risk assessments and identify staff training needs.

You will be responsible for co-ordinating the assessment, planning, implementation and evaluation of care including regular care reviews in conjunction with senior clinical care manager and care consultants.

Delivering and evaluating training courses to internal and external staff. Including the assessment and evaluation of clinical competence in practice for existing and new staff on as annual basis and as clinically indicated.

Ensuring quality of care by developing and interpreting philosophies and standards of care including clinical governance, measuring health outcomes against standards, making or recommending appropriate adjustments to care, conducts and supervises the completion of clinical audits.

Undertaking clinical on-call rota for care, nursing and medical concerns to ensure 24 hour cover alongside your clinical colleagues.

Contributing to the recruitment process of new starters by selecting, orientating, training and mentoring.

To have confidence and professionalism to facilitate multi-disciplinary communication on client care and managerial issues.

Promotes client independence by establishing client centred goals, teaching and counselling client and reinforcing their understanding of disease/illness, medications and self-care skills.

To maintain a safe and clean working environment through adherence to infection control, medication, COSHH, health and safety and organisational polices, procedures and regulations.

To assist in gathering of data for quality assurance, CQC, Safeguarding colleagues raising any concerns to your line manager.

To maintain professional and technical knowledge by attending educational workshops, reviewing professional publications, establishing personal networks and participating in professional societies. Reflecting these learnt practices within our own policies and training contents.

You will maintain a cooperative relationship among health and social care teams by communicating information, responding to requests, building rapport, participating in team problem-solving methods.

To act as a safeguarding lead in the absence of the Registered Manager and Senior Clinical Care Manager.
