Job summary
The
post holder is an experienced Nurse with a minimum of two years experience
working as an autonomous practitioner. They should be working within their
professional boundaries and will provide care for the patients within both their
own homes and care homes as part of an integrated team within practices.
They
will be skilled in initial history taking, clinical assessment, diagnosis,
treatment and evaluation of care; whilst demonstrating a safe, critical
thinking clinical decision-making process and expert care delivery for those
patients who are vulnerable and at risk of avoidable admissions within the general
practice population.
The
post holder will be integrated into practice teams, whilst working collaboratively
with the general practice team members, care home staff, patients, and other
Healthcare professionals to meet the needs of this group of vulnerable patients,
whilst supporting the delivery of best practice, policy and procedures.
In order to work at this level NMC requirements
for advanced practice must be met.
Main duties of the job
- Clinically
examine and assess patient needs from a physiological and psychological
perspective, and plan clinical care accordingly
- Assess,
diagnose, plan, implement and evaluate treatment/interventions and care
for registered patients presenting with an undifferentiated diagnosis and
long-term chronic conditions
- Assess,
diagnosis, plan, implement and evaluate interventions/treatments for
patients with complex needs
- Proactively
identify, diagnose and manage treatment plans for patients at risk of
developing a long-term condition (as appropriate)
- Diagnose
and manage both acute and chronic conditions, integrating both drug- and
non-drug-based treatment methods into a management plan
- Prescribe
and review medication for therapeutic effectiveness, appropriate to
patient needs and in accordance with evidence-based practice and
national and practice protocols, and within scope of practice
- Work with
patients in order to support compliance with and adherence to prescribed
treatments
- Provide
information and advice on prescribed or over-the-counter medication on
medication regimens, side-effects and interactions
