GP Severnside - Clinical Coordinator

Job summary

Step into a senior, daytime leadership role at the heart of IUC. As a Clinical Coordinator (CC) GP, you'll be the on-shift clinical lead: keeping patients safe, supporting colleagues in real time, and helping more people receive the right care in the community.

Where you'll make a difference

  • System CAS - Our multi-professional hub that safely manages most cases in the community, reducing Emergency Department (ED) attendances and 999 calls. You'll help review cases initially flagged by NHS 111 for Category 3/4 ambulance or ED, ensuring only those who need these dispositions use them and that community pathways are offered where appropriate.
  • CMDU - Remote assessment of patients referred for COVID antivirals. You'll confirm eligibility, prescribe oral antivirals where appropriate, and arrange onward referral for IV options if required.
  • SevernSide IUC - Telephone advice, face-to-face care at Treatment Centres in the OOH period and, when needed, home visiting through our OOH teams.
  • Linked services - Support the Weekday Professional Line (WDPL), Frailty Assessment & Coordination (F-ACE) and the Mental Health Integrated Access Partnership when physical health input or prescribing support is needed.

Main duties of the job

  • Assess and manage patients across all age groups in urgent primary care, including physical and mental health needs.

  • Undertake telephone, video, and remote assessments for patients, NHS111 referrals, and Covid Medicine Delivery Service cases, prescribing where appropriate.

  • Support community management, identify emergencies, and ensure safe onward referral or admission when needed.

  • Use System CAS, CLEO, PACCS, EMIS, and other clinical resources to document assessments and guide patient care.

  • Provide advice and support to colleagues, including risk management, prescribing guidance, and safeguarding.

  • Buddy and support new clinicians, including GP trainees, and contribute to on-call rotas during peak demand.

  • Collaborate with clinical and operational teams across CMDU, Severnside, System CAS, Frailty-ACE, and Weekday Professional Line services.

  • Participate in service improvement, quality initiatives, and training programs to enhance patient care pathways.

Essential

  • Full registration GMC GP register
  • Inclusion on NHS England's Performer's list MRCGP or Certificate from the Joint Committee on Postgraduate Training for General Practice

Desirable

  • Experience in training, clinical supervision or coaching
  • Higher post graduate membership/ qualification e.g., MRCP/ MSc

Knowledge and Experience

Essential

  • Experience and interest in working across urgent primary care settings
  • Actively working in General Practice or some form of community medicine or IUC service
  • Effective multidisciplinary team working as part of current or recent primary care experience, either in practice or IUC, or both

Desirable

  • Experience of teaching Medical Students
  • Experience of GP Training/ Clinical supervision
  • Familiarity with the BNSSG healthcare system
  • Acute/ general medical experience
