The role involves drug chart clinical screening, medicines reconciliation and medicines optimisation, patient counselling, and contributing to medication safety audits.
Main duties of the job
You will work as an integral part of the wider clinical team, delivering a high standard of efficient, patient-centred care. You will be expected to adhere to St Josephs Hospital policies and procedures to support the best possible clinical outcomes for our patients.
