Job summary

Sunderland GPA is supporting the ICB in delivery of the Community Mental Health Transformation (CMHT) Programme. A key project within the CMHT programme is the implementation of Community Hubs to support the local community access mental health support.The Community Hub Manager will lead the implementation of the Community Hubs through a project life cycle. The postholder will work with mental health service providers, Primary Care, Voluntary and Community Sector Organisations (VCSO) to ensure the Hubs are integrated with the local population served by each Hub.

The Community Hub Manager will also oversee the day-to-day operational management of the Hubs.

The postholder will work as part of a wider Integrated Neighbourhood Team, ensuring that there is integration with the work activities of the programme and other partners where appropriate.

The employer is Sunderland GPA and the postholder is expected to comply with the GPA's employment, clinical and information governance policies and procedures and any other policies and procedures necessary for the safe and effective discharge of the duties contained within this document.

Main duties of the job

The purpose of the role is;

To successfully implement the Community Hubs in the PCN areas identified

To ensure the that the Community Hubs are fully integrated with the community / VSCO

Provide specialist knowledge, advice, and guidance to partners, stakeholders, and voluntary and community sector organisations to identify gaps in service and facilitate innovative new ways of working and approaches to ensure effective delivery of the aims of the Hubs

To be the Community Hub contact point for stakeholders including the ICB, CNTW, Primary Care, the VSCO and other third sector organisations.

The operational management and development of the Hubs

Complete full evaluation of the hub's effectiveness in line with agreed outcomes to prove the concept to extend funding opportunities for the service.

Day to day operational management of the Hub

Line Management of an administrato

Development of project plans, monitor to ensure milestones are met, identify risks and mitigating actions

Establish relationships with VCSO and partners across all areas

Establish referral routes/processes between the Hubs and other key services e.g. Healthy Heads and Family Hubs

Seek opportunities to link with other key stakeholders / interested parties for further development of the Community Hubs

Attend partnership meetings

Promote the hubs in PCNs to ensure that the relevant practices are aware of the activities and services delivered in the Hubs

Provide monthly progress summary reports to the Portfolio Manager and quarterly reports for the ICB.

Seek funding opportunities and develop bids/proposals to support the Hubs

To manage a performance management framework to measure success of the project

Work in partnership with primary and secondary care partners to support and promote more effective ways of working across the patient pathway

Autonomous working and the ability to manage own time effectively

Ensure all services operate within the organisations clinical and information governance policies, identifying, resolving and reporting, where appropriate, any departures from protocols

To work flexibly within the team to ensure the work of the whole team can be effectively managed

Take a key role in evaluating approaches and implementing new ways of working

Develop and maintain a database of useful contacts

The job holder will carry out any other duties as may reasonably be required by their line manager
