We are
seeking a dedicated and passionate Frailty Advanced Clinical Practitioner (ACP)
to join our dynamic team and work closely with Primary Care Networks, integrating
with the wider community nursing teams. As
a Frailty ACP, you will play a vital role in improving patient outcomes and
shaping the future of healthcare provided to our ageing population.
Frailty is a long-term condition and a
clinically recognised state of increased vulnerability among older adults. With an increasingly ageing population, Primary
Care Networks and Sunderland Community Nursing Teams are joining forces to
develop an integrated frailty team
Advanced
communication, problem-solving and decision-making skills will be used to
assess and provide on-going patient care. Working within local and nationally agreed
protocols and guidelines, the ACP exercises independent judgement to assess,
investigate, diagnose, plan, implement and evaluate the clinical care and
management of community patients.
The post holder will be part of a team that leads non-medical models of care across primary and community-based settings, including care homes and within community teams. The ACP will play a crucial role in step up, step down and anticipatory care through timely assessment, referral, responsive intervention and care planning.
About us
Working as part of a community-based nursing team and closely aligned to Primary Care Networks and General Practice, the successful candidate will be key in leading and delivering the pro-active, personalised care required to patients living with frailty, nurturing collaborative working across Health and Social care to maximise the opportunity for timely identification of frailty in older patients.
Brief Responsibilities:
- Collaborate with Primary Care Networks and a diverse multidisciplinary team to develop and implement comprehensive care plans for frail patients.
- Conduct thorough geriatric assessments to identify the unique physical, cognitive, and psychosocial needs of the ageing population.
- Provide expert clinical assessment and management of complex medical conditions associated with frailty, including falls, polypharmacy, and cognitive impairment.
- Develop and implement innovative strategies to optimize care and enhance the well-being of frail patients, all while considering the impact on the NHS.
- Support primary care practices in identifying and managing patients at risk of frailty, and proactively intervene to prevent complications.
- Foster effective communication and coordination between primary care, community services, and secondary care services to ensure seamless patient care.
- Deliver education and training to healthcare professionals, empowering them with the knowledge and skills to address the challenges of an ageing population.

Work autonomously as an Advanced Clinical Practitioner, across traditional boundaries, within the Frailty Team and with Primary Care Networks and provide a high level of expertise to ensure patient-centred clinical care. They will act as a highly specialised role model for advanced practice, working closely with the multidisciplinary team and other key stakeholders.
Exercise advanced clinical expertise, levels of judgement, discretion and decision making whilst undertaking the skills of assessment, examination to provide a diagnosis and formulate management plans both reactive and proactive within an agreed scope of practice
Evaluate investigations and revise treatment plans according to patient need and assess impact and outcome.
Undertake advanced clinical assessment and examination, including but not limited to:
- Clinical data interpretation, investigation and treatment.
- Ability to develop advanced comprehensive care planning and reviewing and assessment of interventions.
- Diagnosis and disease management and delivery of expert clinical interventions and treatments.
- Non-medical prescribing.
- Assess and treat varied clinical cases, having advanced skills to assess the patient with a differentiated or undifferentiated clinical diagnosis
Work collaboratively within the multi-professional team and support the education and development of both nursing, medical and allied-health professionals.
Lead a wide MDT focussing on ageing well and frailty
The Advanced Clinical Practitioner (ACP) will work collaboratively with their respective Primary Care Network in Sunderland and the wider Neighbourhood Team to meet the needs of patients.
Advanced communication, problem-solving and decision making skills will be used to initial assess and provide on-going patient care. Working within local and nationally agreed protocols and guidelines, the ACP exercises independent judgement to assess, investigate, diagnose, plan, implement and evaluate the clinical care and management of Community patients.
The post holder will be part of a team that leads non-medical models of care across primary and community-based settings, including care homes and within community teams, in collaboration with multi-professional and multi organisational colleagues. Will also play a crucial role in step up, step down and anticipatory care through timely assessment, referral, responsive intervention and care planning
There will also be dedicated allocated time for management and continuous professional development.
