Digital Transformation Lead - Harness North (Part Time)

Job summary

Harness North Primary Care Network (PCN) is seeking a dynamic and experienced Digital and Transformation Lead (DTL) to support the coordination and delivery of high-quality, innovative primary care services. The DTL will play a central role in operational leadership, digital transformation, and service redesign across the PCN, in line with the Network Contract DES, ARRS framework, and NHS priorities.

Main duties of the job

The post holder will lead on key digital programmes across the PCN, including the implementation and optimisation of Abtrace and EMIS systems. They will act as the PCNs digital and transformation lead at local and ICB forums, helping to ensure digital maturity across all practices.

They will also provide operational leadership across the Primary Care Network (PCN), ensuring services are delivered in line with the PCN DES, Investment and Impact Fund (IIF) and evolving NHS priorities. This includes ensuring systems are in place for clinical governance, quality assurance and risk management.

Supporting the recruitment and induction of ARRS staff, as well as provide line management support to team members such as care coordinators.

Additionally they will provide management and administrative leadership for the PCNs Oversight Group, ensuring effective coordination of meetings, follow-up actions, and programme governance. They will be responsible for supporting the development and delivery of the PCNs priorities, ensuring key decisions and progress are documented and monitored. This includes maintaining oversight of project documentation, reporting requirements and communication with member practices and external stakeholders.

Harness North PCN consists of 10 GP practices based over 8 sites, caring for a population of over 75,000 patients.

We work together with a range of local providers, including acute trusts, community services, social care, voluntary sector and patient participation groups, to offer more personalised, co-ordinated health and social care to our local population.

Job responsibilities

The post-holder will:

Lead the delivery and continuous development of PCN services in line with the Network Contract DES, Investment and Impact Fund (IIF), PCN neighbourhood development and evolving NHS priorities.

Ensure timely and accurate submissions for DES, IIF, Enhanced Services and other income-generating initiatives working closely with clinical and administrative teams.

Design and implement new models of care that maximise the impact of the multidisciplinary team and embed digital tools and pathways effectively.

Develop and maintain comprehensive Standard Operating Procedures (SOPs), governance framework and business continuity plans to support business resilience.

Lead the strategic implementation and optimisation of digital transformation programmes across member practices, including but not limited to:

- Cloud-based telephony

- Digital triage platforms

- Abtrace optimisation

- EMIS Hub and workflow standardisation

Represent the PCN at digital transformation forums and other external networks.

Act as the primary liaison with key internal and external stakeholders, including practice managers, clinical leads, ICB digital transformation teams, IT support providers, and external contractors.

Monitor and evaluate the impact of digital initiatives, using data dashboards, feedback, and quality improvement methodologies to inform continuous improvement.

Commit to ongoing professional development, including training in digital health, leadership, service redesign and change management.

Lead the transformation and operational delivery of the Enhanced Access Hub, including digital triage, capacity planning, reporting and continuous service improvement.

Support the design and implementation of Neighbourhood Models of Care, aligned to integrated neighbourhood teams and local place-based priorities.

Facilitate cross-practice collaboration and standardisation of pathways across the PCN for long-term condition management, urgent care access and preventative health.

Work with local partners (e.g community services, mental health providers, local authority teams) to support integrated service models.

Drive patient-facing digital innovations (e.g., online consultation tools, NHS App optimisation, patient education platforms) to enhance access and self-management.

Champion digital inclusion and ensure equitable access to digital services for diverse and underserved populations.

Contribute to the creation and delivery of a PCN-wide digital and transformation roadmap aligned with local Integrated Care System (ICS) strategy.
