Job Title:Health & Wellbeing Coach (Diabetes)
Hours of work:Part-time (22.5 hours per week Monday to Friday)
Remuneration:£27,007.50 - £29,500 (dependent on experience)
Leave entitlement:25 days plus bank holidays per annual (pro-rated for 3 days per week)
Contract length:Fixed term until 28 February 2027 (with potential for extension)

Location:Primarily Haringey GP Federation Offices (48 Station Road, Wood Green, London, N22 7TY)

Main duties of the job
North Central London Integrated Care System partners in Haringey are working together to redesign how diabetes care is delivered for local residents. Although people living with long-term conditions spend less than 1% of their time in contact with health care services, self-management support has historically not been prioritised despite strong recommendations by NICE guidance.
To strengthen support for residents, Haringey is piloting My Way Diabetes, a digital self-management platform designed to help people living with diabetes build the knowledge, skills, and confidence to manage their conditions more effectively. The platform provides more personalised information, educational resources and tools that enable residents to better understand their diabetes and take a more active role in their day-to-day health.
This pilot forms part of Haringeys wider commitment to improving outcomes, reducing inequalities and empowering residents through accessible, evidence- based self-management support.

The overarching aim is to support self-management of those newly diagnosed with diabetes or those with an established diagnosis but at rising risk of hospital attendance/admission or complications. The Health Coach will support people to engage with the self-management modalities on offer, recognising digital exclusion where it might occur and taking steps to find solutions to improve confidence in accessing support and self-management of their condition. The health coach will also support contacts to improve their health in other ways, linking them with services such as social prescribing, healthy cooking initiative Bags of Taste, Prevention services such as smoking cessation, weight management or increasing physical activity.
The Health & Wellbeing Coach will initially support residents in the Central Neighbourhood, working towards our goal of integrated neighbourhood teams. The service will act as a single point of access for diabetes self-management in this neighbourhood and the health coach will be an integral part of this new and exciting approach to working with people with diabetes. The post holder will work with the individual, MyWay Diabetes (a self-management platform) plus primary, community, prevention, VCSE and secondary care services improving self-efficacy/activation and engagement.
The Coach will be upskilled to understand how the MyWay platform works so that they can communicate the benefits and support onboarding especially in those for whom English is a second language, where health literacy is poor or where digital engagement is a barrier
